FTE: 0.8 (64 hours per two weeks)
Shift: 8-hour shifts
Weekend/Holiday: On-call every 4th weekend and 1 holiday/year
Make a Difference in Hospice Care
Emplify Health by Gundersen is seeking a Home Health Aide (CNA) to join our compassionate Hospice team. In this role, you will provide essential care and support to patients and families during end-of-life transitions, ensuring comfort and dignity every step of the way.
Hospice services are available within a 50-mile radius of La Crosse, WI, and you’ll work alongside a dedicated multidisciplinary team that includes nurses, social workers, chaplains, bereavement coordinators, and fellow hospice aides.
What You Will Do
Work 0.8 FTE (64 hours per two weeks)
Provide care during 8-hour day shifts
Participate in on-call rotation every 4th weekend and 1 holiday per year
Deliver compassionate, hands-on care to patients and families during end-of-life transitions
Collaborate with a supportive team to ensure holistic care
What You Need
High School Diploma or equivalent
Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the WI & MN State Home Health Aide Registry upon hire
Minimum of 1 year CNA experience in acute care, nursing home, or home health agency
WI Registered Certified Nursing Assistant upon hire and MN Registered Certified Nursing Assistant within 90 days of hire
BLS CPR certification from American Heart Association or American Red Cross with hands-on learning
