About This Role
As our LVN Home Care Supervisor, you’ll be the steady, supportive leader behind exceptional care in the home. You’ll guide and coach our in-home care team to ensure every participant receives care that is safe, reliable, and deeply respectful—always aligned with PACE standards and personalized care plans.
This is a hands-on, field-forward role where you’ll own the daily rhythm of home care; scheduling, coverage, and service continuity, while providing real-time coaching through ride-a longs, check-ins, and competency support. You’ll also bring LVN-level clinical oversight where it matters most: completing in-home assessments within scope (like vitals and symptom checks), spotting early changes in condition, and escalating concerns quickly to the RN/Provider/IDT.
You’ll work closely with the Interdisciplinary Team (IDT) to coordinate care, support smooth transitions across hospital/SNF/home, and ensure documentation and compliance stay strong—so participants and caregivers experience care that feels organized, consistent, and truly supportive.
