Job Description Summary

The Graduate Nurse (GN) shall be under the discretion of a Register Nurse (RN) or Advance Practice Registered Nurse (APRN), which includes either a Psychiatric Mental Health Nurse Practitioner and/or Family Nurse Practitioner.  Preference is given to Family Nurse Practitioner due to the primary scope of practice.

Graduate Nurse is responsible for clinical, administrative, and clerical duties to ensure that physicians and advance practice nurses are supported, and patients are comfortable. The Graduate Nurse will systematically and continually assess the physical, emotional, and clinical needs of assigned patients.  Graduate Nurse shall be trained in providing medical support to individuals impacted by a variety of behavioral health issues who are learning to manage aspects of their mental illness/substance use recovery.

Responsibilities:

  • Completes triage and initial triage assessment, provides patient monitoring, and documents change in condition in a timely manner; assesses and documents patient status prior to treatment and after treatment.  Triage assessment includes checking a patient’s vital signs, performing a physical exam and obtaining the patient’s medical history. Vital signs include blood pressure, pulse, breathing rate and temperature.
  • Formulates initial plan of care based on assessment of patient needs.
  • Performs coordination of care in collaboration with other behavioral healthcare team members.
  • Provides ongoing patient/family education to include self-care, medication instruction, documents education daily.
  • Accurately documents patient treatment, responses, and maintains accurate account in patient record.
  • Demonstrates professional behavior.  Attends required meetings, communicates and performs job in a professional manner; following policies and procedures.
  • Demonstrates good customer service skills, communicates, and works effectively with others.
  • Graduate Nurse shall report to and perform other work-related duties as assigned by the Director of Urgent Care Services.

Physical Requirements:

  • Typical demands include prolonged sitting, frequent standing, bending, stooping for up to a 12-hour period.
  • Lift/carry up to 10 pounds continuously, occasionally lifting up to 50 pounds, and up to 100 pounds with assistance.
  • Requires normal range of hearing and manual dexterity sufficient to operate keyboard, telephone, photocopier, calculator, and other office equipment as needed. 
  • Sufficient sensory-motor dexterity to complete motor skills tasks, including vision, hearing, speech, and smells.
  • Requires ability to work under stressful conditions and to work irregular hours and deal with irate individuals.
