Key responsibilities:
Schedule member appointments for annual or change-based functional eligibility re-determinations, optimizing time and travel resources.
Use the DHS screening tool to assess members’ functional abilities, health, and required supports.
Collaborate with staff to ensure assessment data aligns with internal records.
Conduct in-home, face-to-face interviews and enter assessment data into the designated online system to determine level of care.
Submit documentation promptly and accurately according to the established guidelines.
Attend meetings and training to stay updated on processes, screening tools, and state directives.
Participate in quality assurance activities to maintain screening competence and reliability.
Maintain required certification through ongoing skills testing, per State of Wisconsin standards.
Demonstrate professionalism and respect in documentation and assessments, preserving screen integrity.
Required Qualifications
Four-year bachelor’s degree in a Health or Human Services related field with one (1) year of experience working with at least one of the Family Care target populations OR be a Wisconsin licensed Registered Nurse with one (1) year of experience working with at least one of the Family Care target populations.
Long Term Care Functional Screener (LTCFS) certification-Selected individual must meet requirements and successfully complete online screener certification training course(s) and become certified as a functional screener.
***The Family Care target group population is defined as: frail elders and adults with intellectual, developmental, or physical disabilities***

Workstyle: This is a field position where employees perform their core duties at non-company locations, such as providing services at business partner facilities or prospects' and members' homes.
Work Location: Columbia, Dane, Sauk Counties
Travel: up to 50% throughout Columbia, Dane and Sauk County, WI and surrounding areas.
Typical Work Days/Hours: Monday – Friday, 8:00 am – 4:30 pm CST
