What you will do:

The Community Hospice has a full-time opportunity for a social worker to join our hospice team caring for patients in homes and nursing facilities as part of our Interdisciplinary Team. As a member of the hospice team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is full-time Monday- Friday 8a-4:30p.

Responsibilities:

  • Diagnose and treat psychological, behavioral and emotional disorders
  • Provide specialized skills and mental health therapy for clients Identify barriers which impact optimal patient functioning
  • Ensure patients receive appropriate resources in most appropriate setting

What you will need:

  • Master's degree in Social Work is required.
  • NYS LMSW licensure is required.
  • Minimum of 2-3 years of experience in Medical, Psychiatric, or Health Care setting

Pay Range: $30.00 - $43.08

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
