The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Social Worker Field Care Manager employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

Position Responsibilities:

  • Manage a caseload and complete comprehensive assessments with members in their home or community‑based settings, as well as telephonically.
  • Provide clinical support and guidance, collaborating with Interdisciplinary Care Team (ICT) participants to support members with medical complexity.
  • Develop and coordinate individualized care plans, ensuring members receive appropriate services to effectively manage their health needs.
  • Identify and address barriers to care while advocating for optimal member outcomes.
  • Review, assess, and complete medical attestations and provide ongoing clinical oversight.
  • Ensure members receive services in the least restrictive setting to achieve and/or maintain optimal well‑being through ongoing assessment of care needs.
  • Develop and modify Individual Care Plans, involving applicable members of the care team (informal caregivers, health coaches, PCPs, etc.).
  • Support members and/or caregivers using an interdisciplinary approach to access social, housing, educational, and other community resources, regardless of funding source.
  • Serve as the primary point of contact for the ICT, coordinating with members, care team participants, and external resources to ensure comprehensive care delivery.

Required Qualifications:

  • Must reside in the State of Michigan with the ability to travel to Wayne and Macomb counties (15–20%).
  • Bachelor’s degree in Social Work (BSW) from an accredited institution.
  • Active Michigan social work licensure in good standing, including one of the following: LLBSW, LBSW, LLMSW, LMSW, or LCSW. Limited license holders must demonstrate intent and active progress toward full licensure.
  • 2+ years of post-degree clinical or care management experience in a behavioral health, medical, or community-based setting.
  • Experience working with adult populations, including chronic conditions and disease management.
  • Knowledge of community health resources, social service agencies, and support programs.
  • Ability to travel to the region-based facilities and in-home to conduct face-to-face assessments with Medicare/Medicaid members, as well as telephonic engagement.
  • Valid driver’s license with reliable transportation.
  • Strong communication and interpersonal skills with the ability to quickly build rapport.
  • Ability to work with minimal supervision and independently within defined role scope
  • Proficiency using electronic medical records and other healthcare-related software systems.
  • Excellent keyboard and web navigation skills
  • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel

Preferred Qualifications:

  • Licensed Master's prepared Social Worker (LMSW/LCSW)
  • Experience with in-home assessment and care coordination
  • Experience with health promotion, coaching and wellness
  • Experience with Medicaid Long Term Care
  • Previous managed care experience
  • Bilingual- Spanish, Arabic or Chaldean Neo-Aramaic
  • Motivational Interviewing Certification and/or knowledge

Additional Information

  • Schedule/Time Zone: Monday through Friday, 8:30 AM – 5:00 PM or 9:00 AM – 5:30 PM Easter Time Zone, with flexibility to work overtime as needed
  • Work Location: Michigan, with the ability to travel to Wayne and Macomb counties
  • Work Style: Hybrid (80% remote / 20% field)
  • Travel Requirements: Up to 20%, including travel to Wayne and Macomb counties

Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher. 

TB Statement: This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$65,000 - $88,600 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
