Job Summary: This position is assigned to the Fleet and Family Support Program (FFSP); Fleet and Family Readiness (FFR); NSA Washington. The primary purpose of this position is to serve as the installation non-clinical case worker for the Exceptional Family Member Program (EFMP). Serves as a liaison to and works closely with health care professionals on provision of available services and resources at the command to ensure best possible care in accordance with community, national and Navy standards.

Major Duties:

Duties include but are not limited to: Implements the EFMP in assigned AOR and conduct ongoing assessments of its consistency and effectiveness. Identifies program needs, challenges and present recommendations to the supervisor for elevating through the chain of command to CNIC and assistance to influence policy compliance issues. Stay abreast of Department of the Navy (DON), Secretary of the Navy (SECNAV), OPNAV, and CNIC policies and interpret/articulate their meaning, priorities and intent.- Utilizes focus groups, in person contacts and surveys to connect with service members and their families to identify resources and services and promote availability of the EFMP to support individual needs. Assists families in assertively pursuing rights to resources available to them. Participates as assigned in the Special Needs Action Team, identifying appropriate community resources, and providing referral information for families. Collects data in accordance with policies and guidance. Participates in coordinating and executing special needs camps, support groups for EFMP families, special events and workshops coordinated by the EFMP. Collaborates with the EFMP team to ensure the program effectively supports families while maintaining compliance with all legal and regulatory guidance. Manage, monitor and support member's progress from enrollment through transition out of the Navy. Communicates directly with each assigned EFMP family upon notification of assignment to offer support services. Interviews and assess individual client and family circumstances prior to and during enrollment to identify and evaluate their special needs and situational stressors, determine services for appropriate assistance, provide tailored guidance and updated information on services to improve quality of life and track progress of activity via appropriate DD Form 3054, "Family Needs Assessment and Service Plan," and/or Inter-service Transfer Summary, as necessary. Maintains a case file in the Navy Accountability and Assessment System (NFAAS) for each assigned EFMP family that includes the Family Needs Assessment and Service Plan and detailed contact notes. Documents and maintain current records in the Fleet and Family Support Management Information System (FFSMIS), as required. Assists service members and their families with accessing available resources and services to support individual needs using warm handoffs and follow-up protocols. Educates and inform families on special education documentation and support services, i.e. school services, access to therapies, related resources, and workshops, etc. Prepares families in advance to attend Individual Education Program (IEP) and/or 504 plan meetings and may accompany parent(s) to the meeting as a support person, maintaining a nonspeaking role. Tracks the trends of the EFMP within the area of consideration (AOR) to include family circumstances, areas needing improvement, effectiveness of activity progress, and provide updated information on services to improve quality of life. Recommends and assist eligible members in connecting with appropriate resources and services. Maintains a detailed and consolidated portfolio of services/resources available for special needs families located on the installation and in the local community. Identifies conflicting or incomplete case information and work with the family to update, providing proper recommendations, and determining status and eligibility of enrollment of members to ensure continuity of care. Researches and analyze trends impacting families with special needs, identify needs that are not being met for planning and assessment purposes, and report and share findings with leadership, management and action groups. Provides education and training on the EFMP to service members and family members, command personnel, and military and community organizations. Develops, update and maintain installation EFMP education, outreach and public affairs programs, to include awareness briefings for commands, units, community organizations, individuals, families and distinguished visitors. Develops and facilitate workshops and trainings on the EFMP and other topics of interest for families and commands. Maintains personal records of required and completed staff training, and professional development trainings completed to comply with program requirements and to gain knowledge to enhance ability to provide up-to-date and relevant service and support. Perform other duties as assigned.

Qualifications: Resumes must include information which demonstrates experience and knowledge, skills, and ability (KSAs) as they relate to this position. A qualified candidate possesses the following: A bachelor's degree or higher from an accredited college or university in behavioral or social science; or related disciplines appropriate to the position; OR Four (4) years of progressively responsible appropriate experience of one or more of the behavioral or social sciences equivalent to a major in the field; OR A combination of education and experience that provided knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. Three (3) years of progressively higher-level graduate education leading to a Ph. D. degree; or Ph.D. or equivalent doctoral degree; OR Three (3) years of progressively responsible specialized experience maintaining case files for families with special needs members; OR A combination of education and specialized experience that provided knowledge equivalent to a subject-matter field appropriate to the position. Comprehensive knowledge of Exceptional Family Member Program requirements and operations, the military lifestyle, and the dynamics between the Exceptional Family Member Program and local community resources for families enrolled in the program to identify the needs of families, trends, and provide recommendations for process improvement. In-depth knowledge of Family Readiness and its mission and programs and practical knowledge of case management practices, processes, and techniques to accurately carry out case management responsibilities. Knowledge of SECNAV, OPNAV, and CNIC policies and procedures to accurately manage cases and interpret policy intent. Practical knowledge of professional verbal communication methods, processes and techniques and of virtual training and meeting platforms to effectively communicate with various individuals and families, train service members and their families on EFMP, coordinate EFMP events and activities, and conduct briefings. Comprehensive knowledge of the Individuals with Disabilities Education Act and other disability civil rights laws, including the Americans with Disabilities Act and Section 504 of the Rehabilitation Act and of military and community support programs to educate families on the resources available to them and assist them in connecting with appropriate resources. Practical knowledge of professional written communication methods, processes and techniques and practical knowledge of Microsoft Office software (PowerPoint, Word, Excel) to maintain case files, update and retain supporting documents, and maintain and develop resource materials.
