The Patient Education team supports DaVita’s Chronic Kidney Disease (CKD) education program. Kidney Smart is a component of The Patient Education Team. The Kidney Smart program is designed to educate and empower patients and their families around understanding and managing Chronic Kidney Disease. Kidney Smart classes are no cost and available to participants across the country.

 

The Patient Education Programs Facilitator will be instrumental in helping to drive volume of CKD participants educated to the program by conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving life-changing CKD education through Kidney Smart.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide direct telephonic and virtual education to CKD participants

  • Provide telephonic call support in focused areas across the country (education scheduling, rescheduling, and reminder calls)

  • Complete data entry & tasking into data management system

  • Provide local coverage support with ops and data entry as needed

Other duties and responsibilities as assigned including but not limited to:

  • Attend team meetings, phone conferences, and training as needed

  • Know, understand, and follow teammate guidelines, employment policies, and department or company procedures

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree preferred
  • Clinical license/relevant clinical certification preferred
  • Skilled in working with a wide variety of participants who may be challenged in hearing, sight, and mobility
  • Skilled in reviewing, educating and answering questions on all modality options for dialysis including, transplant, hemodialysis, PD and HHD
  • Intermediate computer skills and proficiency in MS Teams, Word, Excel, Power Point, and Outlook
  • Experience working and tracking tasks in Salesforce preferred
  • Proven track record of running tactical execution and balancing many tasks under pressure
  • Ability to independently execute on logistical tactics and prioritize appropriately
  • Excellent attention to detail and strong organizational skills

LANGUAGE SKILLS:

  •  Fluent in the written and verbal skills necessary to perform successfully the essential functions, duties, and responsibilities of the position in English.
  • Bilingual preferred.

 

VISION REQUIREMENTS:

  •  Vision adequate to perform essential duties and responsibilities of position.

 

PHYSICAL DEMANDS:

  • Ability to lift minimum of 2 pounds to a maximum of 15 pounds unassisted; able to sit for extended periods of time with some standing, stooping, walking, stretching, reaching; moderate range of body motions.

  • Ability to hear and interact by phone and in person clearly and efficiently; ability to make effective presentations to management at all levels.

  • Ability to use computer and all peripherals for extended periods of time.

 

WORK ENVIRONMENT:

  • Remote Position
  • No required tasks involve greater exposure to blood, body fluids or tissues than would be
  • encountered by a visitor. The normal work routine involves no exposure to blood, body fluids or
  • other hazardous conditions. Normal hazards associated with occasional business travel.

The Wage Range for the role is $19.50 - $25.50 per hour.

 

 

 

If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.

New York Exempt: New York City and Long Island: $64,350.00/year, Nassau, Suffolk, and Westchester counties: $64,350.00/year, Remainder of New York state: $60,405.80/year New York Non-exempt: New York City and Long Island: $16.50/hour, Nassau, Suffolk, and Westchester counties: $16.50/hour, Remainder of New York state: $15.50/hour

Washington Exempt: $77,968.80/year Washington Non-exempt: Bellingham: $17.66/hour, Burien: $21.16/hour, Unincorporated King County: $20.29/hour, Renton: $20.90/hour, Seattle: $20.76/hour, Tukwila: $21.10/hour, Remainder of Washington state: $16.66/hour
