CORE JOB SUMMARY
The Discharge Planner - PRN coordinates all discharge needs for the hospital discharges as outlined. This role works collaboratively with the Social Worker and the Case Manager covering the clinical area.
CORE JOB FUNCTIONS
1. Processes the physician order based on the patient’s choice and based on insurance contract agreements with certified agencies.
2. Coordinates all Home Health, Durable Medical Equipment, Orthotics and Infusion.
3. Utilizes electronic referral system to send referrals for all post discharge needs to licensed providers.
4. Assures follow-up with companies for the coordination of benefits and services.
5. Obtains any required forms for completion with the physician attending, qualified practitioner or ordering physician for completion.
6. Assists the social worker with obtaining patient choice when warranted.
7. Reviews Medical Record electronically to obtain appropriate documentation for post discharge needs and coordinate services.
8. Communicates the finalization, delivery, and acceptance of case to the nursing and care coordination staff.
9. Adheres to University and unit-level policies and procedures and safeguards University assets.

CORE QUALIFICATIONS
Education:
High School Diploma or equivalent/relevant experience, certification or license
Experience:
Minimum 1 year of relevant experience required
Certification and Licensing:
Basic Life Support Certification (BLS)
