Hourly Pay Range:

$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

A Brief Overview:
We are seeking a dynamic and experienced Manager, Clinical Care Coordination to lead our clinical care coordination team. In this pivotal role, you will oversee the care coordination process, ensuring the seamless transition of patients through various stages of care and improving the overall quality of patient care delivery. The Manager will collaborate closely with clinical staff, patients, families, and community resources to optimize patient outcomes and enhance the patient experience.

What you will do:

  • Conduct thorough assessments of patients' medical, social, and emotional needs to identify potential discharge challenges.
  • Collaborate with the healthcare team to determine the most appropriate post-discharge care plan.
  • Develop individualized discharge plans that include recommendations for post-acute care, follow-up appointments, and necessary support services.
  • Ensure care plans are patient-centered and aligned with best practices and regulatory requirements.
  • Facilitate communication among healthcare providers, patients, families, and post-acute care facilities to ensure a seamless transition.
  • Collaborate with community resources and agencies to arrange for home health services, durable medical equipment, and other support services.
  • Educate patients and their families on the discharge plan, including medication management, follow-up appointments, and self-care strategies.
  • Provide emotional support and coping strategies to address patients' concerns and fears.
  • Advocate for patients' rights, preferences, and needs in the discharge planning process.
  • Assist patients in accessing available resources, services, and benefits.
  • Maintain accurate and detailed records of discharge planning activities and patient progress.
  • Generate reports to track patient outcomes and program effectiveness.
  • Participate in quality improvement initiatives related to discharge planning.
  • Identify opportunities for process improvement and contribute to enhancing the discharge planning process.

What you will need:

  • Masters Degree Social Work Required
  • Experience in discharge planning, case management, or social work in a healthcare setting.
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR) State licensure as a Licensed Clinical Social Worker (LCSW) in Illinois Preferred And
  • Certified Case Manager (CCM®) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM or ACM) Preferred Or
  • Accredited Case Manager (ACM) - American Case Management Association (ACMA) Certification in Case Management (CCM or ACM) Preferred
