Discharge Nurse
Days - Full Time

Position Summary / Career Interest:
0900-1700 M-F no weekends, no holidays
Responsible for the delivery of professional nursing services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction, and coordinates discharge planning with other disciplines.
Responsibilities and Essential Job Functions- Performs patient assessment
- Utilizes plan of care
- Validates that patients have clear and concise information and understanding of discharge instructions
- Completes accurate discharge medication reconciliation
- Assesses patient/family learning needs
- Utilizes interdepartmental collaboration according to patient needs to prepare for discharge
- Provides staff education on discharge process
- Ensures safety of patients, visitors, colleagues and self
- May perform Staff Nurse duties as needed
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience- Graduate from an accredited nursing program.
Preferred Education and ExperienceRequired Licensure and Certification- Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with eligibility to work in the state of MO.
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification- Medical/Surgical (RN,C)
- Gerontology (RN,BC)
- Progressive Care Nursing (Adult) (PCCN) - American Association of Critical-Care Nurses Certification Corporation (AACN)
- Certified Medical-Surgical Registered Nurse (CMSRN) - Academy of Medical-Surgical Nurses (AMSN)
- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
