Summary:
The Director - Population Health provides enterprise leadership for the strategy, design, implementation, and optimization of a population health clinical care management model across Memorial Health Network (MHN), Broward Guardian ACO, and affiliated value-based programs. This role oversees value-based payment initiatives, including the Medicare Shared Savings Program and the Comprehensive Care for Joint Replacement Model, supporting clinically integrated providers and hospitals in dual-sided risk contracts across multiple commercial payers. The Director aligns patient outreach and care management to employed and community providers to drive performance and advance technology-enabled, data-driven population health strategies.Responsibilities:
Collaborates with stakeholders to design, implement, and support value-based care programs that improve population health outcomes, quality of care, patient safety, and contractual performance.Develops and deploys education and processes to support chronic condition–specific order sets, evidence-based care plans, and standardized care pathways.Defines, leads, and implements analytics to drive care management team’s navigation of patients throughout the care continuum, leading to more cost efficient and higher quality outcomes.Delivers feedback and recommendations related to care management performance, technology enablement, and analytics advancements to streamline performance improvement and optimize contractual outcomes.Collaborates with Medical Staff Leadership and Chief Nursing Officers (CNOs) to lead clinical transformation initiatives.Serves as a clinical advisor for care management across the healthcare system, supporting alignment of clinical practice with population health and value-based care strategies.Maintains knowledge of the evolving local and national healthcare landscape. Translates regulatory, payer, and market changes into actionable strategies and programs.Manages program managers and other professional staff in the development and governance of multidisciplinary system task forces focused on care transformation initiatives, Skilled Nursing Facility (SNF) navigation and post-acute care management.Ensures alignment of clinical programs with payer contracts, quality benchmarks, and pay-for-performance metrics to maximize outcomes and financial incentives.Leads program development that embeds health equity principles, addresses social determinants of health (SDOH), and establishes infrastructure for community-based referrals and support services for at-risk populations.Competencies:

Complexity of Work: This role requires critical thinking skills, effective communication, decisive judgment, and the ability to build and foster positive relationships. The incumbent must be able to lead others and take appropriate action when required. Required Work Experience: Minimum of ten (10) years of clinical experience in a healthcare setting, including at least three (3) years of leadership experience within a Medicare Accountable Care Organization and/or Commercial Clinically Integrated Network. Experience in ambulatory care management for a minimum of three (3) years is preferred.
