FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy.

Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others.  FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.   

The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.

Family Health Centers of San Diego is looking for a Director, Physical Rehabilitation Services (PRS). The Director, Physical Rehabilitation Services (PRS) provides strategic and operational leadership for a multi-disciplinary department that includes Physical Therapy, Chiropractic, Sports Medicine, Acupuncture, and Weight Management programs across multiple clinic locations. The Director oversees all operational functions, staffing, financial performance, regulatory compliance, quality improvement, workflow optimization, and department-wide growth.

Responsibilities:

Leadership & Workforce Development

  • Builds positive relationships and fosters a high-performance workforce.
  • Recruits, trains, develops, and retains staff across all service lines.
  • Conducts coaching, counseling, performance management.
  • Establishes expectations, workload standards, and career development pathways.
  • Ensures on‑time completion of all training and compliance requirements.

Operational Management

  • Oversees daily operations for all PRS service lines.
  • Ensures efficient workflows for scheduling, referrals, provider efficiency.
  • Partners with Medical, Nursing, BH, Operations, and Patient Engagement.
  • Ensures compliance with Joint Commission, OSHA, infection control.
  • Coordinates facility needs and ensures PRS spaces meet FHCSD standards.

Strategic Planning & Service-Line Growth

  • Develops strategic plans for expansion across PT, Chiro, Sports Med, Acupuncture, WM.
  • Leads new program design, workflow redesign, and access optimization.
  • Evaluates service trends, patient needs, and growth opportunities.
  • Drives innovation including groups, telehealth, and integrated care pathways.

Financial Management

  • Participates in annual budget development.
  • Monitors expenditures and ensures fiscal accountability.
  • Maximizes revenue, productivity, utilization, and efficiency.
  • Ensures compliance with grants and reporting requirements.

Quality, Compliance & Continuous Improvement

  • Leads quality initiatives and standardization.
  • Implements LEAN tools (5S, A3, PDSA).
  • Ensures adherence to quality-of-care standards.
  • Leads service recovery and patient satisfaction efforts.

Community Engagement & Partnerships

  • Builds and maintains community partnerships.
  • Collaborates with Patient Engagement and referral sources.
  • Represents PRS in community and interagency collaborations.

Organizational Collaboration

  • Participates in Director-level committees and strategic planning.
  • Supports marketing, communications, and referral growth.
  • Leads cross-functional integration initiatives.

Requirements:

  • Bachelor’s degree in Healthcare Administration, Business, or related field required. Master’s degree (MBA, MHA, MPH) preferred.
  • 3–5 years progressive healthcare operations experience required.
  • 5 years of leadership experience in healthcare operations for a clinical service line, with responsibility for staffing, workflows, access and performance outcomes required.
  • Ability to manage complex multi-site healthcare operations required.
  • FQHC or multi-site outpatient experience preferred.
  • Skilled in supervising multidisciplinary teams. Experience managing multi-disciplinary programs preferred.
  • Knowledge of budgeting and financial reporting required.
  • Experience leading LEAN/continuous improvement required.
  • High proficiency in MS Office, EHR systems, analytics tools required.
  • Bilingual English/Spanish preferred.
  • Traveling between sites and other locations is occasionally required. Must have a valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency. Reasonable accommodation may be provided on a case-by-case basis. Mileage and other reimbursement governed by policy.

We are excited to share that the salary range for this position is:

91,936.00 - 150,991.00
