As a Patient Educator, you will help coordinate the diabetes education program, providing comprehensive resources to physicians, nursing staff and patients. In this role, you will provide critical education and support on diabetes self-management, including glucose monitoring (with an emphasis on continuous glucose monitoring [CGM]), medication adherence, lifestyle modification and overall disease management. Working closely with the multidisciplinary care team, you will help develop, implement and monitor individualized care plans that promote optimal glycemic control and prevent complications. This position will facilitate organizational change by implementing services that support both individual and system-wide improvement, while delivering high-quality education that enhances organizational effectiveness, increases efficiency and strengthens our ability to meet and exceed patient and stakeholder expectations. It is also vital to maintaining the integrity of our diabetes program, ensuring regulatory compliance, supporting sustainable revenue growth and ultimately improving long-term patient health outcomes.
A caregiver in this role works days from 8:00 a.m. – 5:00 p.m.
A caregiver who excels in this role will:
Provide direct patient care using medical nutrition therapy or diabetes self-management education for assigned patient populations requiring nutrition and/or diabetes support in which medications and monitoring are an integral component in the design of therapy.
Collaborate with Endocrinology providers to design and administer nutrition and diabetes care and education that supports patient safety in the setting of high-risk medication use.
Complete comprehensive assessments for each patient including emotional and behavioral health, knowledge level, disease history, desired outcomes and electronic medical record data.
Develop an individualized care plan with safe and effective interventions based on the patient’s assessed needs and goals and promote successful self-management through education and support.
Utilize the most recent national standards and guidelines for diabetes and obesity care in practice, including the ADCES7 Self-Care BehaviorsTM.
Use diabetes and weight management technology to assess diabetes status, provide patient care and track outcomes.
Deliver specialized interventions and comprehensive counseling and education services to patients with diabetes and obesity who require nutritional management and diabetes care and education.
Implement a care plan, communicate plan, establish a follow-up system (outpatient) and maintain documentation.
Provide complete, concise and timely documentation of each patient encounter in the electronic medical record.
Develop and contribute to Cleveland Clinic sponsored education programs for physicians, nurses, allied health professionals, students, peers and the public.
Use evidence-based guidelines to create and revise education materials related to diabetes and weight management.
Participate in program development designed to market and expand departmental services and revenue generation.
Market services to potential and current referral sources.
Develop and implement services in response to the needs and wants of patients, referring providers and administration.
Actively contribute to departmental initiatives, including committees, projects and community outreach.
Complete reports on ongoing productivity and outcomes tracking.
Participate in professional growth activities.
Attend and contribute to conferences and seminars on diabetes, nutrition and related topics.
Represent Cleveland Clinic on diabetes and nutrition related matters.
Supports departmental research and quality initiatives and apply current research and evidence-based care to practice.
Adhere to department policies, procedures, quality standards and safety standards and comply with governmental and accreditation regulations.
Minimum qualifications for the ideal future caregiver include:
Bachelor's degree or higher in Nursing, Dietetics, Pharmacy or a closely related field
One year of experience as a Registered Dietitian, Registered Nurse (RN) or Pharmacist
Dietitians must have completed a dietetic internship through a credentialed program
Maintain current license to practice dietetics, nursing or as a Pharmacist in the State of Florida
Dietitians must also have active certification as a Registered Dietitian/Nutritionist (RD/RDN)
Earn and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES) within two years of hire/assignment
Preferred qualifications for the ideal future caregiver include:
Connection/experience with diabetes program
Dietician and Pharmacist background
Care coordinator or RN experience
Knowledge with EPIC software
Experience with Diabetes program
Familiarity with CGM (Continuous Glucose Monitoring device)
Physical Requirements:
Ability to navigate extensively to provide services to a large number of patients throughout the hospital and clinic sites. Must have manual dexterity to sufficiently perform electronic data entry, and demonstrate devices and medications to patients.
Must have physical ability to measure and weigh patients.
Personal Protective Equipment:
Follows standard precautions using personal protective equipment as required.
