Position Summary:
To educate patients, their families, and appropriate support systems about diabetes self-management and related issues. The content of this education should include, but not be limited to: pathophysiology of diabetes management (DM), nutrition management, pharmacological interventions, exercise, monitoring, prevention and management of acute and chronic complications, psychosocial adjustment, problem-solving skills, and stress management. To provide for the nutritional care of at risk patients and act as consultant to other health care professionals in determining the best nutritional care options for patients.
ATTRIBUTES
Minimum Qualifications:
• A minimum of two years professional experience teaching and providing nutrition counseling and diabetes self-management training.
• Leadership experience and/or demonstrated skills in group process.
• BS in nutrition management and qualified to provide medical nutrition therapy. Is certified as a registered dietitian by the Commission of Dietetic Registration (CDN). Holds a current certification to practice as a registered dietitian in New York State.
• Excellent communication skills in a variety of settings.
Required Licensure/Certification Skills:
• Is a Certified Diabetes Educator (CDE) or will achieve this status within two years of hire.
• Master’s degree, active exercise trainer certification and/or degree in exercise physiology are strongly preferred.
RESPONSIBILITIES
• Completes a comprehensive nutritional, educational, clinical and psychosocial needs assessment. Interprets appropriate laboratory results and prescribes changes in medical nutrition therapy (MNT) and diabetes self-management training (DSMT). Assesses readiness to change behavior and negotiates goals that promote a healthy lifestyle.
• Develops an individualized MNT and DSMT complex plan of care appropriate to adolescent, adult and geriatric patient populations. Uses a variety of complex interventions and methods of behavioral management.
• Collaborates with other members of the health care team to actively maintain the Diabetes Care and Resource Center as an American Diabetes Association Center of Excellence in providing Diabetes Self-Management Training (DSMT).
• Researches trends in diabetes self-management training. Develops and writes DSMT curriculum that emphasizes patient self-empowerment and self-efficacy. Assumes the role of educator/mentor by providing goal-driven, self-management education and support. Skilled at delivering MNT and DSMT in both group and 1:1 setting. Monitors educational, clinical and behavioral outcomes and revises practice as needed.
• Maintains timely correspondence and required documentation in patient record.
• Maintains certification as a Certified Diabetes Educator (CDE) and Certified Dietitian Nutritionist (CDN).
• Performs other duties as assigned.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
