About the project: PIVOT-TPT
The PIVOT-TPT (Private provider–led Integrated Virtual & Outreach for TPT) is a TIFA (Tuberculosis Implementation Framework Agreement) Health Commitment Grant supported project with PATH providing technical assistance to NTEP (National Tuberculosis Elimination Programme). The project aims to demonstrate a digitally enabled, private provider–led model that expands access to both facility-based and home-based Tuberculosis Preventive Treatment (TPT) services for household contacts (HHCs) of persons with TB (PwTB) seeking care in the private sector, ensuring high initiation and completion rates in alignment with NTEP guidelines.
The project will provide technical assistance to demonstrate across three high-burden districts of Mumbai (Govandi, Sion and Kurla) in Maharashtra and two aspirational districts of Odisha (Kalahandi and Koraput), having substantial gaps in systematic contact listing and TPT initiation in private sector. The model integrates virtual TPT clinics, tele-consultation, standardized digital screening workflows, mobile diagnostic services, and doorstep delivery of TPT medicines to strengthen the identification, screening, and initiation of eligible household contacts on TPT.
The project will operate under NTEP stewardship and leverage national digital platforms such as Ni-kshay and TB Arogya Saathi, engaging private hospitals and clinics, district and state NTEP teams, PPSA partners, digital and logistics agencies, and household contacts of TB patients to ensure seamless coordination and accountability throughout the TPT cascade.
About the position:
The Consultant, under the overall supervision of Deputy Director-TB, PATH, direct supervision of the Project Lead (for Maharashtra) or State Operations Lead (for Odisha), and under the guidance of respective private facility focal providers, will serve as the on-site focal person at assigned private TPT clinic hubs. S/he will manage end-to-end delivery of TPT services including virtual clinic coordination, diagnostics coordination, outreach for drug dispensing and refills, patient adherence monitoring and follow-up, and enrollment and reporting. S/he will act as the primary front-line interface between providers and beneficiaries, ensuring smooth daily operations, accurate data capture for KPIs, and continuous learning from field implementation. A total of 18 Consultants: TPT Clinic Managers will be deployed: 15 in Mumbai, Maharashtra (5 in Govandi, 5 in Kurla, 5 in Sion) and 3 in Odisha (2 in Kalahandi and 1 in Koraput).
Specific roles of this position include:
Serve as the on-site focal person at assigned private TPT clinic hub(s), building strong working relationships with facility focal providers and staff.
Support deployment and operationalization of the virtual TPT clinic tool, ensuring providers are comfortable with appointment scheduling, digital consent, tele-consultation workflows, and e-prescription generation.
Coordinate proactive tele-consultation scheduling for all TB patients notified from the assigned hub, ensuring timely contact with index cases for household contact enumeration.
Support tele-consultation sessions conducted by hub doctors, ensuring beneficiary availability, digital connectivity, and completion of full consultation checklists.
Facilitate enrollment of household contacts in Ni-kshay Contact and Other Risk Group Register, ensuring accurate data entry of beneficiary details, contact information, and relationship to index case.
Coordinate diagnostic services including portable digital X-ray screening, IGRA testing (in Maharashtra), and NAAT testing for household contacts with presumptive TB.
Prepare rosters for digital X-ray field teams, coordinate with radiographers for doorstep screening, and ensure timely upload of results to Ni-kshay and project MIS.
Coordinate sample collection for IGRA/NAAT testing, arrange transport to empaneled laboratories, track turnaround times, and ensure results are shared with providers for clinical decision-making.
Support TPT drug dispensation at facility level, maintaining stock registers, reconciling with NTEP supplies, and coordinating refills for beneficiaries.
Enroll eligible beneficiaries in the doorstep delivery pilot (where applicable), coordinating package handover to e-pharmacy delivery partners and tracking successful deliveries.
Promote digital adherence tracking through TB Arogya Saathi (99DOTS Lite), supporting beneficiary enrollment, training on self-reporting, and monitoring daily drug intake.
Conduct weekly/monthly tele-health checks with TPT beneficiaries to actively screen for Adverse Drug Reactions (ADRs), provide counseling on adherence and side effect management, and escalate concerns to hub doctors.
Conduct outreach visits to beneficiary homes for drug dispensing, adherence support, and counseling where facility-based refills are not feasible.
Provide counseling to household contacts and families on TPT benefits, safety, importance of adherence, and expected treatment duration using standardized risk communication materials.
Record all activities in ODK-based field monitoring tools and project MIS, ensuring accurate and timely data capture for KPI tracking.
Participate in monthly supportive supervision by District Programme Coordinator or State Operations Lead, using standardized checklists to assess hub operations and identify improvement areas.
Support data validation activities including verification of contact enumeration, TPT initiation, and service delivery through patient home visits or tele-calls.
Visit assigned hub facility weekly during initial 3 months and bi-weekly during later months to ensure continuous operations and provider engagement.
Monitor hub-level KPIs including TB notifications, tele-consultation reach, contact identification, diagnostic screening, TPT initiation, adherence enrollment, and completion rates.
Troubleshoot field-level challenges related to beneficiary mobilization, diagnostic scheduling, drug availability, digital tool functionality, and provider concerns.
Document field-level learnings, beneficiary feedback, and provider experiences for continuous improvement and adaptive management.
Support expansion of operations to additional spoke facilities in the assigned geography through provider orientation and coordination support.
Undertake local travel (approximately 60-70% time) for facility visits, beneficiary home visits, and coordination with diagnostic service providers.
Submit weekly activity reports to District Programme Coordinator or State Operations Lead highlighting achievements, challenges, and support needs.
Perform any other project-related tasks assigned by the supervisor.
Qualifications and competencies required:
Graduate degree in Social Work/Nursing/Life Sciences/Public Health or equivalent from a recognized university in India.
Minimum 3-5 years of experience in community health programs, TB programs, or patient counseling and adherence support roles.
Experience in working at facility level or community level with exposure to NTEP operations is highly desirable.
Understanding of TB care cascade, TPT guidelines, and Ni-kshay data entry processes (training will be provided).
Good interpersonal and communication skills with ability to counsel patients, build rapport with providers, and work collaboratively with facility staff.
Basic computer literacy with ability to use mobile-based data collection tools (ODK), WhatsApp, and basic smartphone applications.
Familiarity with digital health tools or willingness to learn tele-consultation platforms and adherence tracking applications.
Strong organizational skills with ability to manage multiple activities, maintain records, and follow up on pending tasks.
Empathetic and patient-centered approach with sensitivity to stigma, confidentiality, and beneficiary needs.
Ability to work independently in facility settings with minimal supervision while maintaining accountability.
Fluency in local language (Marathi for Mumbai positions, Odia for Odisha positions), Hindi, and basic English.
Willingness to undertake extensive local travel for facility visits, home visits, and coordination with service providers.
Comfortable using local transportation and working in urban slum settings (Mumbai) or rural/tribal settings (Odisha).
