Position Purpose
The Community Health Worker assists individuals in the local community gain access to key social service and healthcare providers/services to better their health and improve their lives. This position is based out of Mercy Medical Center.
What you will do
Improve access to health and human services for target population through in-person interactions, telephone contacts, and coordination of referrals
Provide transportation as necessary
Facilitate enrollment in available federal, state and local programs to increase access to health care and support services-- examples include: social security benefits, food, and housing services
Utilize a variety of teaching strategies, including verbal, handouts, and/or video based on participant learning style
Provide basic education regarding chronic health conditions
Minimum Qualifications
High School diploma or equivalent
Current Basic Life Support (BLS) certification
Must possess a comprehensive knowledge of the local community
A current driver's license in good standing and the ability to drive a vehicle or have access to reliable transportation is required
Requires the ability to communicate with target population
Position Highlights and Benefits
Full time 40hrs M-F days

Pay Range: $21/hr
Pay is based on grant-funding
