We are seeking a passionate and engaging Community Health & Wellness Educator to design and deliver programs that empower individuals to live healthier lives. This role focuses on building trusted relationships, promoting behavior change, and connecting community members to vital health resources.
Role Overview
Under the direction of the Community Health Strategies Director, the Community Health & Wellness Educator leads outreach and education initiatives that promote physical, emotional, and overall wellness. This role requires a strong community presence, the ability to motivate diverse populations, and a commitment to advancing health equity.
What You’ll Do
Develop and facilitate health and wellness education programs for community members
Build trust and engage individuals in a variety of community settings
Promote healthy behaviors, including physical activity, nutrition, and emotional well-being
Coach and motivate participants to adopt sustainable lifestyle changes
Collaborate with internal teams and community partners to connect individuals to healthcare and support services
Maintain program data, track outcomes, and prepare reports for leadership
Manage administrative functions including documentation, materials organization, and invoice processing
Support outreach events, including evenings and weekends as needed
Minimum Requirements
High school diploma or equivalent
Minimum of 3 years of community engagement, outreach, or related experience
Strong interpersonal, coaching, and communication skills
Demonstrated ability to engage, motivate, and support diverse populations
Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
Strong organizational skills with the ability to manage multiple priorities and administrative tasks
Ability to obtain Chronic Disease Self-Management Program and Diabetes Self-Management Program certifications within one year
Flexibility to work evenings and weekends as needed
Preferred Qualifications
Bachelor’s degree in a healthcare-related field or equivalent experience
Bilingual (Spanish/English)
