Position Summary:

The Community-Based Pharmacy Residency is a one year (52 week) postgraduate program designed to develop advanced skills in ambulatory care and community pharmacy practice. The resident will provide direct patient care to our health center patients, while gaining experience in medication therapy management (MTM), population health, chronic disease management, and leadership development.

JOB DUTIES & EXPECTATIONS:

  • Deliver comprehensive medication management for patients with chronic conditions such as diabetes, asthma, and complex care needs.
  • Conduct patient counseling, adherence monitoring, and therapeutic interventions in collaboration with the healthcare team.
  • Provide MTM services targeting high-risk populations and support population health initiatives.
  • Participate in specialty pharmacy operations and 340B program compliance.
  • Contribute to quality improvement projects focused on medication safety and chronic disease outcomes.
  • Assist in developing innovative pharmacy services that improve access and equity for underserved populations.
  • Engage in leadership development activities, including committee participation and project management.
  • Lead or co-lead initiatives that advance pharmacy practice within the health center.
  • Pharmacy students serve as role models for professional practice.
  • Provide education to patients, caregivers, and interdisciplinary team members.
  • Deliver presentations on clinical topics and residency research projects.
  • Participate in teaching opportunities for pharmacy students and staff development sessions.

EDUCATION:

· Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution. (required)

EXPERIENCE:

· Eligible for pharmacist licensure in Massachusetts, with licensure required within 120 days of start date (required)

SKILLS/ABILITIES:

· Strong interest in ambulatory care, community health, and underserved populations.

· Excellent communication, problem-solving, and organizational skills

Program Salary: $60,000 anually
