About the Role

 Charlie Health is looking for a motivated and passionate individual to serve our incredible clients as a Care Coach. This candidate will play a critical role in our patients' experience at Charlie Health by supporting them throughout their treatment and beyond into our Alumni services - being their go-to-person when they need the extra motivation and encouragement to understand the importance of being committed to the entire healing process. 

Care Coaches at Charlie Health are individuals who advocate for mental health and are personally passionate about ensuring successful, sustainable healing journeys of those around them. This candidate will be able to empathize with and validate the challenges our clients are navigating. Ultimately, the Care Coach will be their ally throughout treatment, someone who they can depend on to reignite their commitment to treatment and remind them that, oftentimes, the only way out is through

Responsibilities 

  • Oversees assigned IOP group splits
    • Point person for the Care Coaches
    • Reports following each session to supervisor
    • Assigns roles to each Care Coach in group split and manages their schedule
    • Maintains in depth knowledge of each group’s needs
    • Communicates with Primary Therapists about needed accommodations for clients 
  • Subject matter expert on all things related to client groups and cohorts.
  • Exhibits competency in personal recovery and is able to professionally translate their own experience to clients struggling with mental health and substance abuse issues.
  • Engages Charlie Health clients in 1:1 peer discussions via secure platforms to ensure clients are aligned in their treatment goals and accessing all services Charlie Health recommends for treatment.
  • Communicates potential barriers to treatment with admissions and clinical departments and works to identify appropriate solutions to increase accessibility of care.
  • Remains a point person for the client from the first IOP group session through their last Alumni meeting and helps to engage them if their attendance declines.
  • Assists Care Experience and Alumni Coordinators to help create a smooth transition out of Charlie Health.
  • Assists in execution of Alumni services and programming to ensure clients continue to receive the most out of their Charlie Health journey. 
  • Assists with development of internal Care Coach programming and department initiatives.
  • Directly assists Supervisor with projects on an as needed basis.

Qualifications

  • 6+ months in the Care Coach position at Charlie Health or 1 year minimum in similar role at virtual behavioral health company
  • Bachelor’s degree in Psychology, Sociology, Social Work or social/mental health related field and/or relevant certifications (Certified Peer Specialist - national or state accredited).
    • Highly Preferred: Previous expertise working with young adults and adolescents in both individual and group settings is highly preferred (IOP, PHP, or RTC)
  • Excellent interpersonal and communication skills required as well as knowledge of professional board standards as related to client treatment, client rights, and client confidentiality.
  • Eager to work in a fast-paced environment and play a critical role in supporting the success of patient treatment.

Requirements

  • Have access to reliable technology resources (personal phone and computer) & WiFi to work in a remote and confidential setting.
  • Availability 40 hrs/week, including 2 Saturdays per month, and afternoons/evenings on weekdays (10am-9pm MT) to meet the schedules of our adolescent and young adult clients.
  • Authorized to work in the United States and native or bilingual English proficiency. 
  • Proficiency with cloud-based communication and software–Slack, Dropbox, Gmail, Zoom, Google Drive, EMR.
  • Proficient in Microsoft Office–Excel, or Google Sheets.
