Position Summary:
The Complex Care Program provides care coordination and co-management for children with medical complexity due to multiple chronic conditions. The role will meet with patients and families to identify barriers to coping with acute or chronic illnesses/disabilities, while also providing individual, group and family support to patients and their families enrolled the Special Needs program, both in an inpatient and outpatient setting. The role provides crisis intervention as well as long-term therapeutic support for the entire family system.
The role will assess, plan, and implement support plans to meet the overall needs of the entire family. The role empowers patients/families to utilize resources to ensure necessary medical care and to achieve optimal psychosocial functioning. The role will implement and maintain supportive programing within Complex Care to serve the entire family, including but not limited to sibling groups, educational opportunities, inpatient groups and family events. This role will also assist with management of the Family Leadership Committee as well management and educational support of Trauma Informed Care within the Complex Care Program.
