MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Master's Degree in Social Work or related field
2. Must possess current license as required by state board where services will be provided:
MD: Licensed Master Social Worker (LMSW), Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker –Clinical (LCSW-C) through the Maryland Board of Social Work
OH: Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) through the Ohio Board of Social Work
PA: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors
WV: Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through the West Virginia Board of Social Work
EXPERIENCE:

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned.
1. Provides psychosocial assessments of patients and families to identify emotional, social, and environments strengths and problems related to their diagnosis, illness, treatment and/or life situation.
2. Collaborates with all members of the multidisciplinary team to provide comprehensive mental health support for designated caseload.
3. Monitors the patient’s progress by use of validated scales and clinical judgement; intervening as necessary and appropriate, to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective.
4. Provides appropriate brief interventions, psychoeducation, direct care management to patients and families around issues related to the patient’s diagnosis, illness, treatment and/or life situation.
5. Meets with the patient and/or family to assess needs and develop an individualized care plan and to arrange resources to meet the individual’s needs in collaboration with the patient’s care team.
6. Formulates, develops, and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief individual, marital, family and/or group therapies as well as grief and bereavement work.
7. Works with the multidisciplinary team to assist with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence, rape, etc. Provides consultative services to health care team members within scope of care definitions, as needed. Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse
8. Works closely with Primary Care Provider and Collaborating Psychiatrist on identified patients to diagnose, treat and manages mental health and/or substance abuse problems in patients and family members.
9. Documents assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.
10. Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.
11. Maintains a working knowledge of therapeutic techniques and interventions as well as awareness of resources available.
12. Keeps current with social work and health care developments and seeks to increase further enhancements of job related knowledge.
13. Provides teaching and training for students, staff, and faculty from social work and other disciplines. Contribute to the academic mission of the medical center and health system by participating in academic activity, such as committees, conferences, publications, teaching and research.
14. Exhibits professional behavior on a consistent basis.
15. Attends and participates in staff, committee, department, and other administrative meetings.
