Job Description:
Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks
Responsibilities:
Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures
Abstract clinical information from paper/electronic medical records and input data accurately into a quality platform to improve measure performance
Support medication reconciliation post-discharge efforts by sending faxes and calling provider offices/facilities for information
Pull and prep weekly appointment Gaps in Care reports for the Virtual Pharmacy team
Obtain medication lists from provider offices for pharmacist review
Assist with scheduling pharmacist consultations
Pull Likelihood of Hospitalization (LOH) reports and share them with provider offices
Directly outreach to patients to assist in closing gaps in care
Skills and Experience Required:
Bachelor’s degree or equivalent experience
3+ years of experience in healthcare industry
1+ years of clinical chart review/data abstraction experience preferred
Basic electronic medical record knowledge and ability to quickly learn new software
Ability to organize tasks and complete projects in a timely manner
Knowledge of quality measures (ie, HEDIS)
Excellent interpersonal, verbal and written communication skills
Attention to detail and ability to follow precise instructions
Additional Information:
Salary: $20 - $23 Hourly
