The Clinical Pharmacist – Anticoagulation Stewardship supports inpatient pharmacy services across multiple hospitals in the Florida market, serving as a clinical expert and liaison to promote safe, standardized anticoagulation practices. While based at a primary site, this role collaborates across facilities, working closely with pharmacists, physicians, nurses, interdisciplinary care teams, and enterprise pharmacy committees. Responsibilities include providing anticoagulation dosing guidance (e.g., warfarin and heparin), evaluating quality outcomes, identifying gaps in care, and standardizing education, competencies, and best practices across sites. This position blends direct clinical support with strategic program development, offering system-wide impact through strong alignment with quality, safety, and continuous improvement initiatives.

 A caregiver in this position will work Monday through Friday from 7:30am to 4:00pm with no holiday requirements and various weekend requirements from none to every 6th - 8th weekend. This role will be on-site at the primary hospital and provide remote support across the region. Travel requirements occur once per month to other Florida-based CC hospitals.

A caregiver who excels in this role will:  

  • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.  
  • Ensure the appropriate selection, dosing, administration and dispensing of medication orders.  
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.  
  • Document services in a complete, accurate and timely manner. 
  • Actively contribute to medication safety initiatives. 
  • Ensure all pharmacy operations are in compliance with national accreditation standards. 
  • Educate and train students, residents, other caregivers, and/or patients.  
  • Provide cost-effective care. 
  • Be accountable for drug use policy adherence. 

Minimum qualifications for the ideal future caregiver include:  

  • PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.  
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire. 
  • Individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire. 
  • Individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or American Red Cross - or certification obtained within one year of hire. 

Preferred qualifications for the ideal future caregiver include:  

  • Board certification.  
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience.  
  • Clinical Pharmacists hired in a specialized pharmacy practice area: post-graduate year 2 residency (PCY2) or relevant experience.  
  • Prior experience in critical care, pain management, anticoagulation consult services or opioid stewardship (for opioid role)
  • Strong background in acute care/inpatient settings
  • Certifications (preferred but not required): Pain Management (e.g., ASHP certificate)
  • Open to candidates without residency if significant
    relevant experience is demonstrated

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. 
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
