In this role, you will serve as a key pharmacy resource within the infusion suites at the Helen F. Graham Cancer Center, ensuring the safe, effective, and efficient use of medications in an outpatient infusion environment. The ideal candidate will provide expert clinical oversight by confirming medication appropriateness, verifying accurate dosing, and ensuring all required lab values, physical assessments, and line access meet established standards. Through this proactive clinical evaluation, you will help optimize patient outcomes while minimizing medication waste.

Principal Duties and Responsibilities:

Patient Care

  • Review medication orders—preferably prior to insurance authorization—to ensure alignment with clinical guidelines and individual patient needs.
  • Verify laboratory results, patient status, and appropriate venous access before releasing orders for preparation, ensuring all parameters support safe and effective administration.
  • Monitor therapy and recommend dosage adjustments based on patient response and/or laboratory findings.
  • Collaborate with pharmacists and pharmacy technicians within the HFGCC Pharmacy to support timely medication preparation and delivery to infusion suites.
  • Evaluate medication therapies in accordance with organizational policies and best practices.
  • Participate in identifying and managing adverse drug reactions.
  • Provide individualized patient education as requested or based on assessed needs.
  • Support additional infusion pharmacy clinical operations as needed.
  • May require travel to other ChristianaCare locations

Quality Control and Efficiency

  • Develop and implement strategies to reduce medication waste and enhance cost‑effective care delivery.
  • Participate in quality improvement initiatives that strengthen patient safety and care quality.
  • Review and update infusion center medication protocols to reflect current clinical guidelines and evidence‑based practices.
  • Assist with interdepartmental projects, including evaluation and optimization of electronic medical record workflows.
  • Ensure proper handling and storage of medications in compliance with federal and state regulations and ChristianaCare policies.

Documentation and Compliance

  • Maintain accurate and comprehensive documentation of patient interactions, clinical recommendations, and interventions within the electronic medical record.
  • Ensure adherence to all regulatory and accreditation requirements, including those set forth by The Joint Commission, Delaware and Maryland State Boards of Pharmacy, the DEA, and other governing bodies.

Education and Experience Requirements:

  • PharmD degree from an accredited college or school of pharmacy required
  • Current licensure (or eligible to reciprocate) in the State of Delaware required
  • ASHP-accredited PGY1 Pharmacy Practice Residency OR 2 years of experience in an infusion or health-system setting required.
  • Infusion center experience preferred
  • Board of Pharmacy Specialties certification preferred
  • May require pharmacist licensure in additional states

Hourly Pay Range: $58.26 - $93.22

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
