Description:

The Clinical Pharmacist - Ambulatory Care is responsible for ensuring the safe, appropriate, and cost-effective use of medications. He/she provides expertise on drug therapy initiation and monitoring to all customer groups served by Mercy One North Iowa Medical Center, working collaboratively with other health care professionals to devise the most appropriate drug treatment for patients. The role of the clinical pharmacist sometimes extends outside the hospital, sharing their specialized knowledge with other hospitals, clinics, and care settings.

  • Areas of management will include but are not limited to anticoagulation and heart failure GDMT (Guided Direct Medication Therapy).

  • The Clinical Pharmacist will work in partnership with Mercy One North Iowa physicians with a Collaborative Practice Agreement (CPA) to provide drug therapy monitoring according to established policy and procedure. Collaboration will be coordinated with the pharmacists and physicians to access patient’s educational needs, adherence and physical assessments.

  • Performs drug regimen/ laboratory data review and adjusts medication therapy, when appropriate, to maximize outcomes for all Medication Management patients.

  • Provide appropriate patient monitoring to ensure that desired outcomes are met by following Medication Management collaborative practice agreements.

  • Maintains required department confidentiality and abides by HIPPA regulations.

  • Communicates with and performs duties following the philosophy of Mercy One and the Sisters of Mercy reflecting the organization’s Mission, Values, and Guiding Behaviors.

  • Supports and abides by all medical centers, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.

  • Performs other responsibilities as requested by supervisor, manager or department head.

General Requirements:

  • Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.

  • Iowa Pharmacist licensure, either by examination or reciprocity, is required

  • Demonstrated competence in pharmacy for 2-3 years, focusing on the provision of pharmaceutical care is required.

  • Residency training and/or certification in area related to practice site is preferred.

  • Demonstrated competence in previous clinical practice is preferred
