About the role
The Chronic and Complex Care (CCC) service supports individuals living with chronic health conditions to maintain independence and wellbeing at home. By offering targeted and intensive help, CCC aims to reduce preventable hospital stays and help people leave hospital safely and on time.
The service is provided up to a 12-week period, during which clients receive comprehensive support through MDT discussions, linkages with primary health networks & private providers, clinic follow-ups, remote patient care monitoring, maintenance support, and exercise classes to aid in their health journey.
Ensure patients have access to the right healthcare services and community resources to support improved health outcomes and quality of life.
Provide education and practical strategies to develop self-management skills and confidence, enabling patients to effectively manage their health condition within the community setting.
Work in conjunction with the MDT to meet the goals of the patient (and carer) receiving community-based chronic disease services in both a clinic and home-visiting environment.
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
Relevant post-registration qualifications and at least three years’ experience working in the clinical area of their specified post-graduate qualification
Current unrestricted NSW Driver’s License
Demonstrated ability to work within the interdisciplinary team at a senior level
Ability to provide chronic respiratory input, health coaching and education to clients in multiple clinical settings including within their own homes
Previous experience providing assessment and treatment to patients with chronic heart failure or experience managing patients with a cardiac background
