Job Title: Clinic Nurse Manager

Schedule:  8-hour day shifts, 8-4:30 pm, Monday-Friday                               

SUMMARY: Responsible for the clinical management of assigned PPM Practice(s) servicing patients in and around SLH in St. Lawrence County, New York.  Works collaboratively in dyad relationship with Practice Manager(s)/Coordinator(s) in the supervision and support of clinical care coordinator’s, LPN’s and MA’s. Collaborate with key function leaders at SLH such as Director of Quality, Director of Patient Safety, and Director of Patient Experience.  Responsible for the management of quality clinical standards for employed and contracted physicians, mid-levels (providers) and staff.

RESPONSIBILITIES:

Regulatory and Quality Management

  • Supervises and assures effective, high quality clinical activities within the medical office home.

  • Cooperates with all providers and clinical staff in performance of all nursing staff functions in compliance with SLH P&Ps, PPM P&Ps, protocols, guidelines for Clinical Standards of Care.

  • Periodically evaluates and provides ongoing improvement of Delivery of Care standards for all outpatient populations.

  • Performs regular quality audits to ensure satisfactory clinical outcomes.

  • Monitors meaningful use related to computer/software utilization, and provides nurse training in meaningful use and other software applications.

  • Conducts regular clinical practice rounding with a focus on quality, DNV and DOH audits

  • Promotes a culture of safety in the delivery of patient care. · Acts as primary resource for achievement and compliance with all accreditation standards of Det Norske Veritas (DNV).

  • Acts as primary resource for achievement and compliance with all CMS, State of New York Department of Health rules and regulations on the clinical delivery of healthcare.

  • Acts as primary PPM educator for clinical staff on all DNV standards in collaboration with PPM leadership team.

  • Provides effective coaching, feedback and developmental action plans to health site leaders and staff for all aspects of DNV.

  • Monitors all sites for compliance with DNV standards and recommends and monitors corrective actions as needed.

  • Acts as PPM participant on all appropriate SLH committees for DNV, quality, compliance, CMS, etc.

  • Reports PPM compliance with regulatory standards to appropriate SLH departments.

  • Develops actions plans in collaboration with SLH hospital departments as needed.

  • Develops and maintains appropriate Clinical standards Policies and Procedures (P&Ps), Standard Operating Procedures (SOP’s), protocols, guidelines, check lists, etc.

  • Performs periodic review and revision of P&Ps. Acts as expert resource in up-to-date clinical standards and education appropriate staff on changes and updates.

  • Be a resource to all PPM physicians and staff for best practices initiatives, maintenance of confidentiality standards (HIPAA, etc.).

  • Adheres to SLH and PPM Policies and Procedures.

  • Responsible for assuring accurate and timely entries of pertinent medical information on all patients including telephone encounters, reports from outside facilities, preventative services, prescription refill requests and health related concerns.

  • Develops plans as necessary to ensure the successful clinical quality operations of the facility.

Clinical Management

  • Participates in hiring, onboarding, evaluation, and disciplinary activities for all clinical support staff.

  • Works in didactic relationship with Practice Managers and Providers.

  • Oversight of Clinical Care Coordinators and RN’s within outpatient practices.

  • Is knowledgeable about all elements of appropriate 1199 SEIU United Healthcare Workers collective bargaining agreements.

  • Ensures that all providers and staff members are adequately trained in the use of the EMR and monitor compliance.

  • Conducts regular staff rounding to provide clinical support and identify staff growth opportunities / needs.

  • Participates in regular practice staff meetings. · Works in collaboration with SLH and PPM departments as warranted.

  • Integrates SLH mission and core values within clinical services responsibilities.

  • Assists in the investigation and resolution of patient complaints / issues with provider and clinical staff with a focus on quality and patient-centered care.

  • Provides a professional environment where the customer/patient/provider/staff are valued.

  • Maintains open and active communication with entire PPM management team.

  • Acts as a Super user of EMR to support clinical operations, clinical workflow improvement, basic troubleshooting and staff/provider support.

  • Coordinates with PPM management team and serves as primary medical and staff resource for all clinical concerns.

  • Collaborates with PPM Operational team in the ordering and maintenance of medical equipment, pharmaceuticals, and instruments.

  • Establishes, implements and monitors all aspects of nursing activities in support of physicians and mid-level providers, nurse practitioners (NPs), and physician assistants (PAs).

  • Ensures access to appropriate patient education resources via multiple modalities such as educational handouts, internal support resources, and external patient self-management resources and collaborates with clinical leaders and members of the health care team to make resources available to patients. Example: diabetes, CHF, COPD education, the State of New York Vaccines for Children (VFC) program.

Development

  • Development of innovative clinical healthcare programs that support the needs of the St. Lawrence Health System patient population. Ex: Chronic Care Management, Transitional Care Services, Patient-Centered Care.

  • Monitors and analyzes the effectiveness and appropriateness of processes and resources necessary for the care management of patients and patient populations.


Finance

  • Participates with VP of PPM on the preparation and management of the operating and capital budgets of PPM.

  • Contributes to the effective management of medical equipment and supplies in a fiscally responsible manner.

  • Participate in financial reporting and budgeting of facility needs.


Oncology Department:

  • Standardize Infusion practices across the SLH system in coordination with pharmacy, outpatient practices, inpatient units.

  • Assist inpatient Nursing Leadership with elements of planning, organizing, controlling, climate setting, leadership, and operational duties as it pertains to the delivery of chemotherapeutic and biological agent administration in the inpatient setting.

REQUIRED QUALIFICATIONS:

  • Graduate of accredited school of nursing with BSN required (may be waived for individual with diploma or associate degree who demonstrates experience in management of clinical staff.).
  • Current, unrestricted New York State Registered Professional Nursing license.

PREFERRED QUALIFICATIONS:

  • Oncology experience, certification, and management experience preferred
  • Minimum 1 year of supervisory experience; healthcare setting preferred
  • Strong leadership, team-building, and employee engagement skills
  • Proficient in Microsoft Office (Outlook, Excel, Word)
  • Knowledge of eClinicalWorks, CPT/ICD-10 coding, and medical office practices preferred
  • Project management and Lean Six Sigma experience a plus
  • Experience with Patient-Centered Medical Home desirable

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
