Job Description:
POSITION SUMMARY:
The Community Care Network LVN provides ongoing support and education to assigned patients under the direction and supervision of a Registered Nurse. Care is delivered following a plan established by the physician and the Care Management Director, in conjunction with the patient/family and in compliance with the California Vocational Nurse Act. The LVN works within the framework of the Community Care Network team. Directly assists patients and practitioners to navigate the health care system at an optimal level, supports care management activities and ensures patients receive the right services at the right level of care.
POSITION QUALIFICATIONS:
Education/Licensure/Certification:
- Current CA Licensed Vocational Nurse (LVN) license is required.
- Certification in geriatric nursing or geriatric care management preferred
- Current Basic Life Support (BLS) Certification required.
Knowledge:
- Minimum of 5 years acute care LVN or related/equivalent experience is preferred
- Relevant experience in Care Management
