Job Summary :
The Health Information Management (HIM) Clerk supports inpatient and outpatient care by ensuring the accurate collection, maintenance, and secure distribution of patient health information. This role is responsible for prioritizing, locating, assembling, and electronically tracking medical records requested by clinical and administrative departments in accordance with organizational policies and regulatory standards. The HIM Clerk also performs essential clerical and filing functions to maintain the integrity, accessibility, and confidentiality of the Health Information Management Department’s records and workflows.
This position reports to the Revenue Integrity Supervisor.
Job Description
Essential Responsibilities
- Maintains strict compliance with HIPAA regulations by safeguarding the confidentiality of protected health information.
- Ensures that required authorizations are obtained prior to starting and/or completing a record request.
- Reviews, processes, and fulfills MyChart medical records requests in an accurate and timely manner.
- Conducts daily rounds to retrieve medical records for recently discharged patients, including historical medical records, from multiple hospital units.
- Sorts, scans, and indexes medical records obtained from the floors and/or external sources – including paper charts, CDs, and electronic media- into the electronic medical record (EMR) system.
- Organizes and stores medical records according to established retention schedules and ensures proper destruction once retention requirements are met.
- Sorts incoming mail and distributes to the appropriate individuals or departments.
- Answers incoming phones calls, monitors voicemails, and reviews and responds to incoming faxes.
- Identifies and resolves scanning discrepancies including deleting, relocating, or renaming documents as needed.
- Reviews death certificates to ensure accurate and timely updates to patient EMRs.
- Uploads new payor audit documentation into audit tracking software.
- Collaborates with HIM Analyst to identify and resolve missing chart documents and communicates with departments as necessary.
- Assists with processing medical records to support continuity of care for VA patients.
- Uploads and indexes various documents into Epic upon request.
- Reviews and processes EpicCare Link requests in a timely and accurate manner.
- Performs additional duties as assigned.

Education/Experience:
- Must have a high school diploma or equivalent. Prior experience in healthcare or provider clinic is strongly preferred.
Licenses or Certifications:
- Strongly preferred certifications: CHRI
