Make a meaningful impact beyond the bedside. We are seeking an experienced Registered Nurse to serve as the chronic disease management and prevention expert for our Community Health Strategies team. This role focuses on improving population health through education, outreach, culturally competent care coordination, and health coaching—helping individuals and communities reduce risk and better manage chronic conditions.
What You’ll Do
Lead the coordination and implementation of community-based health education and outreach programs focused on chronic disease prevention and symptom management
Integrate access to care through culturally responsive outreach, enrollment strategies, and advocacy
Provide health coaching using motivational interviewing and solution-oriented approaches to support wellness goals
Assist community members in navigating healthcare systems and connecting to needed services
Collaborate with internal teams and external community partners on nutrition, nourishment, and wellness initiatives
Utilize community health needs assessment data to inform outreach strategies and program development
Participate in evening and weekend programs, events, and community initiatives as needed
Minimum Requirements
Bachelor of Science in Nursing (BSN)
Current Missouri RN license
5–7 years of experience in program and/or project management
Excellent interpersonal, written, and verbal communication skills, including public speaking and presentations
Demonstrated expertise in coaching, motivational interviewing, and behavior change strategies
Ability to work independently, prioritize effectively, and manage multiple initiatives
Knowledge of community and public health nursing practices
Proficiency in Microsoft Office applications
Flexibility to work evenings and weekends as required
Valid Missouri or Kansas Class E driver’s license (if driving is required)
Preferred Qualifications
Bilingual (Spanish/English)
Certified Diabetes Educator
Wellness Coach, ACSM HFS, and/or CHES certification
