Job Summary:
This position supports the professional and leadership staff of Chorus Community Health Plans (CCHP). This role includes supporting quality improvement, member engagement, case management, health promotion and population health programs by coordinating related projects, conducting member outreach, and creating internal reports to benefit members served by CCHP.
Essential Functions:
Assists teams in identifying program opportunities related to addressing drivers of health that support organizational and patient/member goals.
Supports leaders with projects and data information needs using analytical software tools, which may include care management documentation platforms, peer reviewed journals and research, and local publically available data sources.
Supports and creates reports, including creating data tables and visualizations and executive/project summaries as needed.
Utilizes data to interpret program efficacy and makes suggestions to leadership based on results.
Performs internet research to support various project initiatives, as directed by leadership staff.
Supports development of member outreach and engagement strategy for quality improvement initiatives.
Develops workflow diagrams, scripting, and evaluation tools to support quality improvement initiatives.
Education:
Currently enrolled in an undergraduate or graduate program Required
Experience:
Office experience and/or healthcare experience or volunteerism Preferred
Knowledge, Skills and Abilities:
Effective oral and written communication skills.
Excellent organizational and problem solving skills.
Ability to handle many concurrent tasks, meet deadlines and work well under pressure.
Ability to adapt to requests from various project owners and leaders.
Have proficient knowledge in all Microsoft products specifically Excel with desired experience creating workbooks, Pivot Tables, and visualizations.
Comfortable self-managing and progressing multiple ongoing and ad hoc projects in a remote work environment.
Knowledge of and experience with office procedures, particularly in a remote setting
