7310 HospiceThe primary purpose of a Chaplain is to provide appropriate spiritual care to the patients, families and staff of Kaweah Health utilizing the resources available including the local faith community, local social services organizations to meet the needs of those served.QUALIFICATIONS
License /Certification
Preferred: Certifications related to Chaplaincy including Thanatology, Grief, Critical Incident Stress Management.
Education
Required: Bachelor’s degree, preferably in spirituality, theology, divinity, religious studies or a related field of study.
Preferred: Master's degree in a ministry related field of study. Completion of three units of Clinical Pastoral Education (CPE).
Experience
Required: Five years of professional experience working with people in times of tragedy, crisis, and emotional upheaval.
Preferred: Ordained and endorsed minister, priest, rabbi or a member of a religious order or lay person with certified training in pastoral care.
Knowledge/Skills/Abilities
Familiar and comfortable working with varied spiritual/religious practices.
Able to work in an ecumenical and multi-faith setting.
JOB RESPONSIBILITIES
Essential
Keeps the patient and family as the primary concern of the contact made on behalf of Chaplain Services. Each contact is based on the needs of the patient and/or family.
Develops professional working relationships with physicians, nurses and the leadership of the organization.
Serves people of all faith and attempts to link patients with their local faith community.
Ministers staff as needed or requested. This may include physicians, nurses, staff and administration.
Facilitates faith community leaders by providing resources as needed.
As part of the health care team, makes daily rounds and provides spiritual and emotional support to the patient.
Is familiar with The Joint Commission requirements related to Spiritual Care and provides timely and accurate documentation in the medical records as required.
Addendum (essential for specific dept)
HOSPICE:Functions as a member of the Hospice Interdisciplinary Team. Provides an assessment of the patient and family spiritual needs. Provides spiritual counseling to meet these needs in accordance with the patient's and family's acceptance of this service, and in a manner consistent with the patient and family beliefs and desires. Makes all reasonable efforts to facilitate visits by local clergy, pastoral counselors, or other individuals who can support the patient's spiritual needs to the best of its ability. Advises the patient and family of this service. Documents all services provided in an accurate and timely manner in the patient's record. Facilitates Patient Memorials for Hospice staff as appropriate. Completes a competency evaluation annually as evidenced by a field evaluation or case study.

May serve on Kaweah Health committees as requested within the scope of the Chaplain role including but not limited to the Bio-Ethics Committee.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.
Performs other duties as assigned.
