Summary

This role provides the opportunity to work with a wide variety of clients in their home environment, focusing on functional safety, Activities of Daily Living (ADLs), and Instrumental Activities of Daily Living (IADLs). You will work under the supervision of a licensed Occupational Therapist to help clients maintain independence and achieve their personal goals.

Home Health COTA (Certified Occupational Therapy Assistant)

Full-Time | Monday–Friday | No Weekends or Holidays

Position Overview:
We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our Home Health team. This role provides the opportunity to work with a wide variety of clients in their home environment, focusing on functional safety, Activities of Daily Living (ADLs), and Instrumental Activities of Daily Living (IADLs). You will work under the supervision of a licensed Occupational Therapist to help clients maintain independence and achieve their personal goals.

Responsibilities:

  • Provide home-based occupational therapy interventions as directed by the supervising OT

  • Assist clients with ADLs and IADLs to promote safety, independence, and quality of life

  • Conduct functional assessments and document progress

  • Collaborate with the OT, caregivers, and interdisciplinary team members

  • Educate clients and families on adaptive techniques, safety, and equipment use

  • Maintain accurate and timely clinical documentation

Schedule & Environment:

  • Full-time, Monday–Friday

  • No weekends or holidays

  • Travel to client homes required

Qualifications:

  • Valid Minnesota COTA license

  • Valid driver’s license and reliable transportation

  • Strong communication and interpersonal skills

  • Ability to work independently in a home setting

  • Experience in home health or geriatrics is a plus (optional to keep)

Qualifications

Appropriate education level required in accordance with state licensure.



Must be licensed as an Occupational Therapy Assistant in the state(s) of practice. When working at a Sanford Health Facility (not required but preferred for GSS Facilities): Must also be certified as an Occupational Therapy Assistant through the National Board for Certification in Occupational Therapy (NBCOT). Conforms to the supervision and licensing standards of the state(s) where services are provided. When applicable, may require valid driver's license. Basic Life Support (BLS) certification required within six months of employment.
