The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

$86,964.80 - $113,672.00

Position Overview

ESSENTIAL FUNCTIONS

  • Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences, and initial and period assessments
  • Supervises clerical staff, social workers, rehabilitation staff, recreational staff, chef/ kitchen staff, supply chain, facility management and center aides.
  • Facilitates and leads morning meetings of the interdisciplinary team (IDT).
  • Facilitates and leads regularly scheduled Care Plan meetings to review participant cases on a regular basis.
  • Oversees the quality of care and safety of PACE participants, both at the PACE center and in the community.
  • Serves as a liaison between disciplines and between administration and direct staff.
  • Works closely with the medical care team as part of the IDT to develop and implement participants individualized care plans.
  • Coordinates 24-hour care delivery.
  • Coordinates with the marketing department to hold site tours for new candidates at the PACE Center.
  • Serves as a representative of Capital Health LIFE/PACE to the community.
  • Coordinates activities related to new participants.
  • Manages the centers budget.
  • Oversees overall documentation cohesiveness and accuracy in each participant’s medical record.
  • Utilizes PCO/RTZ appropriately and efficiently and serves as content and workflow expert for resolving issues and proposing solutions in PCO/RTZ.
  • Performs other job duties as required.

MINIMUM REQUIREMENTS

  • Bachelor's degree or higher in a clinical related field required. Acceptable qualifications include, but are not limited to: Physical Therapy, Occupational Therapy, Nursing (BSN or MSN), Social Worker (MSW).
  • Three years experience in a leadership role within a healthcare setting. Minimum one year of experience working with older adults and their families in a healthcare or community agency setting similar to PACE is preferred. Experience working as an Interdisciplinary Team Member or supervising members of an IDT.
  • Drivers License (this position may involve home visits to LIFE participants if needed)
