CDAMS-GMC Key Worker

  • Flexible hours. 1.4 EFT Fixed term 1/4/26-30/6/26

About the Role
Exciting opportunity available to gain experience within a sought-after role in memory clinic and geriatric medical clinic. Part time and full-time roles are available, dependent on applicant preference. You will work within a flexible, multidisciplinary structure, that ensures the delivery of high-quality assessment and management services to clients, making sure that client and family needs are met in a timely manner. 

Successful candidates will be mentored within a supportive workplace. Memory clinic experience is valued but not necessary. Applicants will need to be passionate about working with patients with memory loss and older populations.

You will conduct triage, comprehensive pre-clinic assessments, develop the client’s care plan and provide ongoing education and service linkage for clients and families as appropriate. You will be the patient and family's contact person throughout their journey.

The role is currently based at St George’s Hospital, Kew.


Classifications for this position will range from the below dependent on qualification:

YY7 Grade 4A Registered Nurse ($107,962.40 per annum pro rata)

SC21 - SC24 Grade 2 Social Worker ($90,235.60 - $103,724.40 per annum pro rata)

VF6 - VF9 Grade 2 Occupational Therapist ($90,235.60 - $103,724.40 per annum pro rata)

VW1 - VW4 Grade 2 Speech Pathologist ($90,235.60 - $103,724.40 per annum pro rata)

Your Contribution

  • You will have a Bachelor of Allied Health discipline (e.g. Social Work, Occupational Therapist, Speech Pathologist or equivalent, or a Bachelor of Nursing)
  • You have strong assessment skills and case management skills
  • You have experience or are passionate about working with clients with dementia / geriatric conditions
  • You have good knowledge of the community supports available for clients with dementia
  • Current National criminal history check, or willing to obtain

About CDAMS and Geriatric Medical Clinics - Health Independence Program (HIP)
The CDAMS & Geriatric Medical Clinics are part of the HIP Specialist Clinics. Services are provided in the centre. The role involves interaction with GPs, medical specialists, clients, family and carers, other SVHM and HIP services.
