Position Summary

Position will provide youth-centered case management/referral services for high school age youth as part of a school-based health center (SBHC) located at Jennings High School. The position will coordinate with other behavioral health/health staff members of the SBHC multidisciplinary team. The position will also coordinate closely with school faculty and leadership to implement services within the school setting.

This team will create care plans to include identification of social, mental health, health prevention and educational/vocational needs. This position will be dedicated to and experienced in working with diverse youth/families of all races and sexual orientations and identities. The position will utilize assessments and screening to identify areas of concern for youth and provide community resource referral and coordination, crisis intervention and supportive counseling to assist the client/family, and coordinate access to needed resources.

Will abide by the same strict standard of patient confidentiality that applies to all WashU staff.

Job Description

Primary Duties & Responsibilities:

  • Conduct a culturally sensitive comprehensive psychosocial assessment with the client, and family/support network as appropriate, in order to develop a plan of care to meet identified needs.
  • Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to carry out the plan of care. Remove access barriers by referring clients/families to public benefits programs as eligible.
  • Provide regular contact with enrolled clients and document all contacts and service coordination activities in client records as required by program guidelines.
  • Assist youth to secure needed medical follow up/services and monitor kept appointments.
  • Maintain current knowledge of and incorporate evidence-based interventions for youth, and routinely interact and network with community providers, clinicians, and others involved in youth service delivery.  Advocate for improved services for youth.
  • Work in coordination with other SBHC staff and Jennings High School staff to develop and implement a range of youth-centered programming that will address primary issues impacting the lives of youth clients. Co-facilitate needed programs and/or work with community partners to develop/provide needed services.
  • Partner with SBHC staff to work from a youth empowerment/developmentally appropriate framework to assure that youth are active partners in all aspects of the services received at the center.
  • Perform indirect, non-clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff meetings and community meetings as assigned to meet reporting requirements.

Additional Duties:

  • Work collaboratively within the multidisciplinary SBHC team, Jennings High School faculty and administration and community partners.
  • Act as point person for the coordination of behavioral health services and work in partnership with the Project ARK/The SPOT Director, and Medical Director to implement the project; participate in leadership activities and meetings. 
  • Attend meetings as scheduled to coordinate, advocate and plan for youth services.
  • Perform indirect, non-clinical tasks related to the job, including regular attendance at staff meetings and timely compilation of client data upon request.

Salary Range

$52,600.00 - $78,900.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
