Bring your clinical expertise and your passion for patient advocacy to a role where you’ll shape outcomes across the entire continuum of care. As a Case Manager at Atrium Health Wake Forest Baptist, you will be the go‑to resource for the interdisciplinary team—guiding evidence‑based decisions, coordinating services, and ensuring each patient’s journey is timely, seamless, and centered on quality. In this position, you’ll analyze systems, drive improvement, and collaborate daily to align care plans, utilization, and reimbursement with organizational goals—so patients receive the right care, at the right time, in the right setting.

Essential Functions

  • Advance optimal outcomes by planning, coordinating, and evaluating case management interventions for defined patient populations using the nursing process and current research.

  • Facilitate appropriate length of stay and reimbursement for all admissions in alignment with organizational goals and regulatory expectations.

  • Serve as the key information and education resource for physicians, nurses, assistive staff, therapies, pharmacy, dietetics, psychosocial disciplines, infection control, fiscal services, performance improvement, and other contributors to patient care.

  • Promote quality of care through collaboration with patients, families, and all team members, emphasizing continuity, effective communication, and coordination of services.

  • Analyze systems and variances to identify opportunities for improvement; develop organization‑wide approaches to problem solving and throughput.

  • Manage patient flow and discharge planning, removing barriers to timely transitions to appropriate levels of care.

  • Apply a scientific, evidence‑based approach to evaluate patient care/outcomes, quality of life, and cost reduction opportunities supported by current literature.

  • Lead daily (Monday–Friday) case management collaboration to meet expected outcomes; maintain availability when off campus via pager, digital phone, and email.

  • Champion objectivity and advocacy, ensuring equitable access, clear documentation, and adherence to standards, policies, and departmental procedures.

  • Support the mission, vision, and values of Advocate Medical Center, contributing to enterprise goals and performance improvement initiatives.

Education

  • Graduate of an accredited School of Registered Professional Nursing (RN).

  • BSN preferred; MSN encouraged.

Experience

  • Minimum of five years of clinical experience caring for the assigned patient population.

  • Prior case management or utilization review experience encouraged.

Licensure & Certification

  • Current Registered Nurse (RN) license in the state applicable to the job location.

  • Case Management certification (e.g., CCM, ACM‑RN) encouraged.
