As a Case Manager in our homelessness service, you’ll walk alongside individuals experiencing crisis, helping them stabilise their housing, strengthen their wellbeing and reconnect with community. Build strong, empowering relationships that support choice, dignity and self‑determination
Deliver high‑quality case management aligned with TSA models and best practice
Complete thorough assessments of needs, risks and vulnerabilities
Develop and implement collaborative case plans that break down barriers and drive progress
Link individuals with the right supports and help them connect to community
Participate in networks, working groups and Communities of Practice
Bring emotional intelligence, self‑awareness and adaptability to every interaction
Engage in supervision, case reviews and ongoing development
Degree in social work, welfare, community development or related field OR strong sector experience with demonstrated capability
Experience in client assessment and case management within social services
QLD Working with Children Check (role includes overnight accommodation support)
Current Driver’s Licence
First Aid Certificate (if required on roster)
