About the role 

As a Youth Case Manager,  this is your opportunity to work within a team of outreach workers across the eastern region of Melbourne. They will work within the Journey to independence model of care and psychologically informed practice framework to provide a high level of support and case management to young people aged 16-25 years who are homeless or at risk of homelessness to access stable, safe and affordable accommodation. 

 

Reporting to the Client Services Coordinator, this is a permanent part time role offering 30.4 hours a week located in Ringwood, VIC. 

 
How you will make an impact  

  • Client referrals are assessed in accordance with program criteria and ongoing assessment and review of client needs, issues and circumstances from initial intake to end of support period 

  • Clients receive multiple weekly contacts with safety/risk plans developed including mental health and suicide risk assessment, MARAM etc. 

  • Individual case, safety and or private rental action plans are developed in collaboration with clients and include a range of interventions 

  • Clients assisted to access support services and establish community / specialist supports 

  • Maintain strong commitment to working within a trauma and psychologically informed environment 

  • Ability to represent TSA in strategic working groups, network meetings and complete weekly youth focused outposts across the easter region of Melbourne 

  • Case notes are to be completed within 24 hours of each client contact and documents uploaded to TSA information systems 

  • Ensure clients are provided with information pertaining to The Salvation Army and our commitment to the safety and inclusion of LGBTIQA+ people 

 

What you will bring 

  • Relevant tertiary (Bachelor or higher) qualification in the area of Social Work, Social Welfare or related field 

  • Demonstrated experience with accompanying highly developed skills in working with persons experiencing homelessness and who display a range of highly challenging behaviours (essential) 

  • Knowledge of service systems, in particular youth, housing and homelessness, Child Protection, AOD, Family Services and Mental Health and Life and Living Skills 

  • Experience in applying Case Management principles and practice 

  • Demonstrated skills engaging homeless persons and willingness to assist in provision of support in areas of domestic violence, drug & alcohol, mental health, living skills and parenting 

  • A Current Victorian Driver's License and an Employee Working with Children's Check 

  • Compulsory code of conduct, child safe and cultural competency learning modules to be completed and kept up to date

Conditions  

Compensation is in accordance withSocial, Community, Home Care and Disability Services Award Level 5.
