About Journey to Social Inclusion (J2SI)
J2SI is an innovative and evidenced-based case management service for people experiencing long-term homelessness. It differs from traditional homelessness services as it provides the time needed for people to access their permanent home and have an improved quality of life and increased community connection. Staff have the ability to work intensively with small caseloads and use a combined key worker and team approach to deliver intensive individualised support for up to 3 years to each client.
About the role
As a Case Manager, this is your opportunity to provide long term case management and service coordination with clients who have experienced chronic homelessness through the J2SI program. You will ensure that the service provided is supportive, effective in achieving client goals, program outcomes, and are of the highest quality in accordance with TSA’s values and legislative and funding requirements.
Reporting to the Team Leader, this is a permanent full-time role, located in Frankston, VIC.
How you will make an impact
Manage an outreach caseload providing intensive case management, service coordination, referral and advocacy using persistent engagement, trauma-informed practice, risk assessment and safety planning to achieve client goals and program outcomes.
Apply the Case Management Framework and J2SI measurement and evaluation tools to monitor progress, meet targets and maintain accurate, timely and professional client records in line with policies and procedures.
Work collaboratively with ACCOs, First Nations workers, referral partners and local services to deliver culturally safe support, coordinate case conferences and support effective transition and exit planning.
Actively participate in organisational and consortium activities including team meetings, service planning, communities of practice, research and evaluation, and implementation of the TSA Strategic Plan and Model of Care.
Contribute to a safe, reflective and high-quality practice environment by engaging in supervision and professional development, supporting continuous quality improvement, managing information securely and upholding OH&S and risk responsibilities.
What you will bring
A Tertiary qualification in Social Work, Psychology, Occupational Therapy or a related field (essential)
Demonstrated case management experience in the community service sector
Proven ability to restore relationships with people experiencing vulnerabilities and history of complexities in areas such as adult homelessness, alcohol and other drugs, mental health, disability, and family violence.
An understanding of theoretical frameworks and strategies for working with vulnerable people, with proven ability to manage challenging behaviours, respond to crises and achieve program targets and outcomes.
Experience in assisting clients to navigate different service systems to meet their needs.
Knowledge of recovery principles and the ability to apply them in broader casework practice.
Specialist experience in mental health and/or alcohol and other drug settings, with strong knowledge and application of trauma-informed practice.
A current VIC drivers licence and employee Working with Children’s Check
