Your Opportunity

Are you passionate about supporting people to overcome challenges and achieve independence? We’re looking for a dedicated and experienced case manager to join our team and deliver high-quality, client-centred support to individuals with complex needs. This is an opportunity to play a meaningful role in transforming lives while working within a supportive and purpose-driven environment.

Working with diverse populations, this role will provide comprehensive, person-centred case management support to our clients, experiencing or at risk of homelessness, living in our short-term accommodation. Working collaboratively with clients to develop tailored plans, connect them with right services and support their journey towards independence.

Your key responsibilities: 

  • Deliver holistic, strengths-based case management aligned with best practice frameworks
  • Develop and review individualised case plans using SMART goals
  • Conduct client assessments, intake, and referral processes
  • Support clients through transitions to independence or alternative services
  • Facilitate group programs and training sessions tailored to client needs
  • Build and maintain strong relationships with internal teams and external stakeholders
  • Contribute to program reporting, continuous improvement, and service development
  • Maintain accurate client records and ensure compliance with relevant standards

Requirements for Success

  • Relevant Degree with minimum 1 year experience
  • Experience working with clients with complex needs in a case management setting
  • The ability to deliver evidence-based or therapeutic interventions
  • Strong communication and relationship-building skills
  • Excellent organisational and administrative abilities
  • Sound knowledge of compliance and statutory requirements
  • A proactive mindset with the ability to work independently and use initiative
  • A genuine belief in people’s capacity to grow and achieve positive change
  • Valid NSW Driver’s Licence

Before starting work with us, you will need to undertake a national police history check, qualifications, referee checks, and WWCC.
