Your Opportunity

Mission Australia is seeking skilled and passionate Case Managers to join our team in Tweed Heads, Lismore and Grafton.

This is a meaningful opportunity for professionals experienced in working with vulnerable children, young people and their families.

In these roles, you will provide trauma-informed, early intervention case work. You will deliver a mix of long-term and short-term support services, including school and/or home-based case management, needs assessments, practical assistance, referrals, and group-based interventions. You will also participate in community outreach supporting broader awareness and access to services.

We’re looking for people with a strong experience working with in child and adolescent mental health, trauma-informed practice, and family-focused care, who are confident engaging with diverse communities, including Aboriginal and Torres Strait Islander and CALD families. Your key responsibilities will be to:

  • Deliver trauma-informed, client-centred case management to children, young people (0–18), and families experiencing or at risk of mental health issues.
  • Respond to referrals, conduct initial needs assessments, and provide ongoing support through home visits and outreach services.
  • Develop and implement individualized case plans, provide practical support, and facilitate referrals to internal and external services.
  • Coordinate and deliver therapeutic and psychoeducational programs, including Play Therapy Groups and mental health education.
  • Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry.
  • Work collaboratively with internal teams and external agencies to advocate for clients, participate in case reviews, and support community development.

 Requirements for Success:

  • Tertiary qualifications and/or relevant experience in the human services industry, with an understanding of mental health.
  • Provide tailored case management through comprehensive assessments, developing individualized case plans, and facilitating ongoing support for clients.
  • Respond to client referrals promptly, ensuring timely contact, support, and effective service delivery to children, young people, and families.
  • Coordinate and deliver individual and group support programs, such as Play Therapy Groups, mental health education, and community outreach.
  • Excellent oral and written communication skills, with the ability to maintain accurate case documentation and engage effectively with clients and external agencies.
  • Current NSW driver’s license
  • Current First Aid and CPR Certificate

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
