EMPLOYMENT RELATIONSHIP: Just cause subject to probationary period
CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The starting time for this position is currently 8:30 a.m.
GENERAL SUMMARY
Under direction, provides telephonic linkage and referrals to individuals seeking Macomb County Community Mental Health (MCCMH) services. Provides crisis intervention and advocacy for persons served. Provides care coordination. Works in coordination with agencies and community providers to ensure timely and accurate service provision.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Working within a call center environment, completes brief screenings, for populations served by MCCMH including persons with mental illness (MI), substance use disorders (SUD), and intellectual/development disabilities (I/DD) to provide linkage and referrals to MCCMH services.
Schedules intake assessments with MCCMH services.
Provides crisis intervention and advocacy for persons served.
Completes case documentation in accordance with policies and procedures, including clinical records and statistical reports.
Provides care coordination for the completion of MCO duties.
Works with multiple agencies and community providers to ensure timely and accurate service provision.
Maintains and updates clinical knowledge by completing in-services and trainings.
Serves on Community Mental Health or related community-based committees.
Assists in the measurement of effectiveness of interventions to determine outcomes and identify needs of the person served and process improvement.
Must be available to work flexible hours, including evenings and weekends, and on-call rotations.
Operates an automobile to perform assigned job functions.
Performs related duties as assigned.
QUALIFICATIONS
Required Education and Experience
Bachelor’s degree from an accredited college or university in Psychology, Social Work, Special Education, Counseling or other mental health related field
Preferred Education and Experience
Master’s degree from an accredited college or university Psychology, Social Work, Special Education, Counseling or other mental health related field from an accredited college
One (1) year of experience working in a behavioral health setting
Required Licenses or Certifications
Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel
