The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below. In addition, specific functions may change from time to time:
Interviews patients and/or their families upon admission or during evaluation
Compiles an assessment of the patient's history and presenting problems, strengths and needs, systems interactions and anticipated discharge needs
Collaborates with the multidisciplinary treatment team to identify ongoing discharge/planning needs
Contributes specialized knowledge of community resources
Creates linkages throughout the patient's stay between needed community resources, the patient and/or family, and the treatment team, to facilitate continuity of care after discharge
Communicates activity and progress through active participation as a member of the treatment team, timely documentation in the patient's medical record, and compilation of statistical reports
Documents final after care plans and facilitates timely communication of medical records information after discharge
Participates in Quality Improvement activities as needed
Provides crisis intervention as needed
Engages in peer review activities to determine compliance with policies and regulatory requirements
Maintains current knowledge and awareness of community resources for social, emotional or financial assistance
Provides information, education to patient/family on community resources and options for post-acute care appropriate to the patients served. Initiates referrals to community agencies as needed. Ensures patient is provided choice of post-acute facilities per HIPAA regulations

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:
Bachelor’s degree in Social Work or related field required
Previous similar experience with psychiatric population preferred
Pre-degree experience relevant to the position requirements will be considered
Knowledge of the principles, practices, and techniques of social work and social work services as they apply to a developmentally varied patient population undergoing psychiatric treatment and rehabilitation
Must demonstrate good interpersonal and interviewing skills, as well as excellent oral and written communication skills
Must have the knowledge and skill to analyze large amounts of complex information
Must have the ability to respond efficiently and flexibly to rapidly changing demands of rapid admission and discharge patterns, working relationship with multiple treatment teams and coordination of services over a multi-state area
Must have a working knowledge of basic computer skills to include the use of Microsoft Word, Microsoft Outlook and internet browsing and search retrieval
Must be able to see, hear, read and have manual ability to use telephone
Must effectively communicate orally and in writing with patients, family members, personnel and physicians
