Provides case management support to clients of House of Mercy’s residential service program. Duties include but are not limited to coordination and
oversights of: client admission process, orientation to program, treatment schedules, insurance reviews, participation of treatment related programming, groups, enrollment into essential community programs, collaboration without providers for referral and/or discharge needs. ​

Essential Key Job Responsibilities: 

  • Orient new residents to program guidelines and expectations. 

  • Establish rapport and develop a therapeutic relationship and provides weekly psychoeducation group sessions and individual sessions with assigned clients. 

  • Coordinate admission process with new clients through compassion and empathic manner.  

  • Coordinate the timely enrollment of clients in treatment supportive community programs (e.g. insurance, food stamps, FIP, etc). 

  • Act as a resource to clients and staff with regards to community service and supports. 

  • Provide accurate information on a client’s behavior and progress for use in the treatment plan review.  

  • Documentation is completed and filed in a timely manner.  

  • Provide support with discharge process to achieve a smooth transition from House of Mercy to the community.  

Minimum Qualifications: 

  • Bachelor’s degree in Social Work or human service related discipline from an accredited four-year program preferred 

  • CADC certified with the state of Iowa or certified within nine (9) months from date of hired.  

  • Child and Dependent Adult Abuse Mandatory Reporter training to be completed within 30 days of employment. 

  • Valid Driver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle Safety Standards, excellent communication skills.
