At NewYork-Presbyterian, our mental health professionals have expertise in the identification and management of various mental health issues and in developing community minded and effective treatments that contribute to the quality of life and health of our patients. We have specialty programs dedicated to the care of adults of all ages challenged by mental health issues, major mood disorders, psychosis, anxiety disorders, substance abuse, and cognitive disorders (such as Alzheimer's disease and other memory disorders). Our team includes board-certified psychiatrists, clinical psychologists, social workers, nurses, and others with experience in the care of adults.

Case Manager – Adult Outpatient Behavioral Health – Columbia 

Our team is seeking a Case Manager to support our patient population.  Serve as part of multi-disciplinary team providing systems care coordination, targeted referrals and case management, and supportive engagement for patients presenting with emergent mental health needs. The Case Manager will be essential in facilitating community integration and continuity of care by ensuring patients have enduring ties to their community and support systems during critical periods. This position may require field visits in the community, inlcuding providing  time-limited in-home services to patients in need. The team is seeking candidates with strong patient engagement skills, efficient organizational skills, and investment in team work.  Bilingual Spanish speaking candidates are encouraged to apply. 

The schedule for this full time position will be as early as 8am and as late at 7pm. It is a 7.5 hour shift, Monday through Friday.

Preferred Criteria

  • Some training in behavioral health/psychology
  • Fluency in both English and Spanish

Required Criteria

  • Bachelor of Arts or Sciences Degree in Social Work, Psychology or equivalent from a recognized college or university

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

$58,500-$79,000/Annual
