Summary:
The Assistant - Case Management provides support to the case management leadership team in the areas of utilization review, discharge planning, and other related tasks. This role is essential in ensuring the smooth operation of case management functions, facilitating efficient workflows, and supporting the delivery of high-quality care.Responsibilities:
Performs other duties as assigned to support the goals and objectives of the case management department.Collaborates with case managers, healthcare providers, and other team members to facilitate smooth transitions of care.Provides general support for utilization review and/or discharge planning efforts, including coordinating with external providers and resources as needed.Monitors and tracks case status as instructed, ensuring timely updates in case management processes.Assists in identifying and resolving any gaps in processes or documentation to enhance efficiency and effectiveness.Assists case management leadership with day-to-day administrative tasks, as assigned, to support utilization review and/or discharge planning activities.Communicates effectively with internal teams and external stakeholders to address case management needs.Supports the preparation and maintenance of reports, data, and metrics related to case management activities.Follows up on pending tasks, including updating faxes, obtaining required documentation, and ensuring compliance with policies and procedures.Competencies:

Complexity of Work: Able to prioritize and manage multiple tasks and deadlines effectively. Requires critical thinking and problem-solving skills to address challenges in tasks assigned within utilization review, discharge planning, and workflow processes. Involves handling sensitive and confidential patient information with a high level of professionalism and adherence to HIPAA and other legal standards. Demands strong interpersonal and communication skills to collaborate with various stakeholders, resolve coordination issues, and ensure alignment across departments. Able to adapt to changing priorities and work effectively under pressure in a fast-paced healthcare environment. Requires attention to detail and accuracy to identify discrepancies, complete documentation, and support the case management team effectively.
