About Department

Specialist clinics provide over 100,000 non admitted patient appointments per year across 23 different medical and surgical specialties. Specialist Clinics provide a gateway to the hospital for patients requiring elective surgery and ongoing chronic disease management. Our major referral base is via General Practitioners from all areas of Victoria. This is a fast paced environment where teamwork is essential.

About the Role

An exiting opportunity is now available for a Case Coordinator, can be a Registered Nurse or Enrolled Nurse to join the team on a Permanent Full Time basis, 40 hours per week (part time will be considered).

The Case Coordinator under the direction of the Nurse Unit Manager is responsible for coordinating patients across the continuum of care from receipt of referral to discharge from clinic, while ensuring compliance with the St Vincent’s Hospital Melbourne (SVHM) Specialist Clinics Access Policy. A key function of this role will be implementing a case coordination framework for patients referred for a Specialist Consultation.

Classifications for this role will be RN28 ($110,614.40 per annum pro rata).

Duties

  • Coordinate referrals for Specialty Clinics in accordance with the Specialist Clinics Access Policy
  • Run designated clinics including coordinating interpreters, transport patients and correctional patients
  • Ensure outcomes are completed and patients are discharged as per access policy 
  • Monitor and adhere to KPIs
  • Attend department meetings and actively contribute to relevant committees
  • Audit waitlists every six months
  • Participate in continuous improvement activities

Qualifications, Skills and Experience

  • Bachelor or Diploma of Nursing, which meets the registration requirements of the Nursing and Midwifery Board of Australia
  • Current Registration with the Nursing and Midwifery Board of Australia through AHPRA
  • At least 3 years postgraduate acute nursing experience
  • Knowledge and experience in a clinic environment with an understanding of waiting list and discharge planning principles
  • Demonstrated advanced negotiation, problem solving and analytical skills
