Case Coordinator

Responsibilities:

  • Thoroughly reviews, assesses, and completes third party release recommendations related to family reunification options to inform customer’s release decisions.

  • Makes recommendations for most appropriate level of care within the customer’s system of care and facilitates transfers when appropriate.

  • Visits customer-funded care provider sites within geographic area of assignment to conduct weekly case staffing meetings.

  • Confers with customer staff and direct care providers to discuss cases employing perspectives from child welfare, juvenile justice, and mental health in adherence with customer policy.

  • Conducts interviews with children when necessary to support safe and timely reunification or to assess for individual placement needs.

  • Submits weekly reports to customer on case progress and tracked/elevated concerns and trends.

  • Assists care providers and customer to recognize potential victims of trafficking.

  • Provides technical assistance to customer grantees related to customer policies and procedures.

Location:

This is a telework position, while you may cover operations in your vicinity, this position is intended to provide nationwide coverage on a remote basis.

  • Position requires fingerprint background checks and child abuse / neglect checks.

  • Candidates will need to successfully clear Tier 2 Public Trust background investigation for this role.

Education and Experience:

  • MA/MS in Social Work or a social science related discipline +2 years postgraduate child welfare experience, or +3 years of post-undergraduate child welfare experience, or

  • BA/BS in Social Work or a social science related discipline + 5 years post-degree child welfare experience, or

  • BA/BS in a non-social science related discipline and +7 years of post-degree child welfare experience

  • Bilingual proficiency in English and Spanish (will be tested)

  • Prior experience working with immigrant youth and families is highly preferred (case management, psycho-social assessments, direct mental health therapy, etc.).  Juvenile justice experience can be a differentiator.

Schedule: 

Working schedule parameters projected to be M-F 8-5 with some degree of flexibility for weekend and/or evening hours required of position holder. 

Travel: 

Some local and/or distance travel maybe required for his position.  There may be distance travel requirements monthly.

The likely salary range for this position is $54,704 - $73,600. This is not, however, a guarantee of compensation or salary. Rather, salary will be set based on experience, geographic location and possibly contractual requirements and could fall outside of this range.
